1025507712 2017-08-31 Completion Testing No Violations Status check inspection on the well completion process found a service rig on location owned by Fortis Energy Service. The rig is drilling out the completion plugs using 2.875 PH6 tubing. The company is also using ROC Service Company flow back service along with Bass Energy Completion well head control. The product is being flowed into a closed top gas buster 500 barrel tank with the sand being removed by a Sand X recovery system. The company is Fairing any excess gas created. 2.875 N80 will be set as production tubing at a depth of 9,024. All supportive equipment is placed on polymer containment. I also discussed with the company men on site the section of the ORC Rule 1501 9-1-08 section (N) regarding annular pressure monitoring.
1428469674 2018-05-14 Production Wells No Violations Status check inspection found the well in production and selling off the production tree with 3 primary production valves open along with the (ESD) emergency shut down valve. The company has installed on the 9.625 and 13.375 well head sections annular pressure gauges and flow lines that are tied into the low pressure recover system, the company is monitoring all annuli. There is a total of 4 wells on the pad all with the same similar construction with all 4 wells producing through sand trap separators during the initial production stages. There is also 4 gas processing units on location. Also present is a production fluid storage facility consisting of a total of 8 steel construction tanks of 400 barrel capacity located inside a polymer lined steel containment area.

1622918158 2017-01-05 Casing String Witnessed No Violations When I arrived on location Express had not arrived on location yet. When Express arrived on location they got all of their hoses hooked up to begin pumping. As we started to pressure test and run our fresh water gel ahead, they had an issue getting water into the pipe where the cement came into the truck. Express then had to rig down and fix the issue. In the end they replaced the whole pipe and then began to pump cement into the hole. I was on location to witness Express Energy cement the 26 inch diameter conductor casing into a 32 inch diameter borehole. Express drilled a 100 foot 32 inch diameter borehole and set 100 inches of 26 inch diameter conductor casing into the hole. Express ran 9 barrels of fresh water gel ahead of the cement. Express then ran 330 sacks of cement, 2% CaCL, and .50 parts per second of floseal with 110.68 barrels of fresh water at 15.6 pounds and a yield of 1.18. Express then circulated with 55.74 barrels of fresh water until we got cement to surface. Express circulated about 18 barrels of cement to surface and finished cementing the conductor into the bore hole.
1679156411 2017-06-06 Completion Testing No Violations Status check inspection on the well completion process found CalFrac Pressure Pumping Service rigging up on the hole and preparing to begin the stimulation process within the next 48 hours. Nine Energy Wireline will be providing wireline services. The well head consists of a Cameron B section pressure rated at 15,000 pound per square inch (psi) working pressure with a Cactus Completion stack consisting of a lower manual and hydraulic upper master control valves topped off with a flow cross valve section all rated at 15,000 psi working pressure. All stimulation equipment is situated on polymer containment with the well pad perimeter earthen berm being the primary containment.
2114638637 2017-11-01 Preliminary Restoration No Violations Preliminary Restoration status check inspection found the area affected by the well pad construction to have vegetation and erosion control present. At the time of my inspection contract workers from Myers Well Service were on site removing the erosion control filter socks from the well pad perimeter. Also noted is the well pad perimeter has a total of 6 catch basins with fluid outlet discharge drains in the closed position. Addressing the 4 active wells on the location, all 4 wells have been drilled and completed and the initial flow back stages are complete. The wells are complete with production trees and the production outlet valves are in the open position. The tin horn cellars have been filled with river gravel and have siphon tubes in place for fluid recovery. The mouse holes have also been closed in. There is annular monitoring in place on the well heads. The wells are selling into gas processing units with a total of 4 units present on location one each supporting each well. The well production fluid is being discharged into a tank battery consisting of 8 steel tanks holding a capacity of 400 barrels of fluid each, downstream from the tanks are 2 low pressure vapor burner stacks. The well pad and lease road are of stone construction with signage posted at the well pad entrance and a gate present half way down the lease road.
