0486242582 2017-08-16 Completion Testing No Violations The pad is named Switz 26 and consists of 4 wells. During my inspection Universal Pressure Pumping, Allied Wireline, and CNX Company Man were on location. Universal Pressure Pumping was rigged up for hydraulic stimulation operations on the C and A well. Operations were at a stand-still and there was not any equipment in operation. I discussed with CNX Company Man annular pressure monitoring. He stated that they monitor the annular pressure at the wellhead and then run a line to the frac van when operations begin to record and monitor their pressures. The wellhead was equipped with a frac stack with all valves in the close position. Annular pressure monitoring was present. A 2 inch diameter steel line was plumbed up to the 9.625 inch diameter casing and ran to a bled off tank.
0523988128 2018-02-21 Production Wells No Violations Status check inspection found the well drilling and completion process now complete and the well in line and producing from the tubing flow cross section. The casing section of the well head is isolated with no flow lines attached. All well head annulis have pressure gauges and flow lines present with check valves present on the flow lines. On location is also a production fluid tank battery consisting of a total of (4) tanks of steel construction design situated inside a polymer lined steel containment area. Also on location is 4 gas processing units on each servicing each well on the pad. EQT also has a midstream operation off the back corner of the well pad. Emergency Shut down devices are located at the well pad entrance-at each well head -the tank battery and the gas processing units.
0688006282 2017-08-03 Completion Testing No Violations The purpose of my inspection was a status check. During my inspection the well was equipped with a frac valve in the closed position. The pad surface was lined with containment liner and frac tanks were being placed on containment. Location was in preparation for hydraulic stimulation operations.
1493252547 2017-04-26 Casing String Witnessed No Violations The purpose of my inspection was to witness the surface-mine string cement job. The coal seam is from 623 feet to 639 feet in depth. Patterson Drilling Rig 803 drilled a 24 inch diameter borehole to a depth of 713 feet. The 18.625 inch diameter casing, 87.5 pounds per foot, was run to a depth of 687 feet. Allied Cement Services was on location to cement the casing string in two stages with a packer set at 409 feet. I witnessed the last casing joint being landed. A pre job safety meeting was held with all personnel on location. Allied pumped 150 barrels of fresh water, 50 barrels of gel, and 10 barrels of fresh water were pumped ahead of the cement. 78 barrels of Class A cement with 1.5 percent calcium and 207 barrels of displacement were pumped to complete the first stage. The plug landed and the packer was energized. The second stage was completed using 190 barrels of fresh water, 50 barrels of gel, 10 barrels of fresh water, and 137 barrels of Class A cement with 1.5 percent calcium. 125 barrels of displacement were pumped, the floats held, and 15 barrels of cement returns were witnessed in the cellar. Cement density from both stages was checked during the job using pressurized scales confirming the weight of 15.6 pounds per gallon.
1503594621 2017-08-24 Completion Testing No Violations On site inspection during the well stimulation process found 4 wells on location and the company currently stimulating the (A) well on location identified as 34111247210000. The annular pressure of the (B) well was indeed verified prior to stimulating the 21st stage on the (B) well. The annular pressure was blead down from 400 psi to 36 pi and remained consistent at 36 psi throughout the stage.
1558013652 2018-05-21 Production Wells No Violations Producing well status check found that a total of 4 wells are present on the well pad and in production with the production valves on the tubing head assemblies in the open position. There is a total od 4 gas processing units on location and 4 steel design production tanks. A 3rd party midstream operator identified as EQT is set up at the rear of the well pad. There is a large portable water storage tank used in the stimulation process set up on the well pad also.
