0258983266 2018-08-15 Completion Testing No Violations Status check inspection on the well completion process found the well drilling -stimulation and completion process is now complete. 2.375 diameter EUE tubing has been set in the well bore. 4 additional wells on the pad will have the plugs drilled out and tubing set in the well bore.

1034747667 2018-05-14 Completion Testing No Violations Status check inspection on the well completion process found US Well Service pressure pumping on location along with Allied Horizontal wireline service. The 1st stage to be completed through the 3 toe sleeves set in the production casing string have been complete and while attempting to set the wireline bridge plug for the 2nd stage on the evening of 05/12/2018 there was a electrical storm that moved through the area and the wireline crane along with 3 of the pressure pumps were affected by the storm. The wireline gun was in the hole and the completion bridge plug was set at a depth of 21,645 and the 1st stage perforated between 21,647 and 21,645. At that point the storm disturbance occurred and the 110 wireline crane became disabled with the boom unable to function. At the time of my inspection a second crane has been brought in and Allied Wireline was preparing to run in the hole with a perforating gun and continue the perforating schedule on the said well. Crews were on site attempting to repair the crane so it may be rigged down and moved off site. The crane is standing at full mast unable to operate.
1105734401 2018-09-10 Completion Testing No Violations Status check inspection fount the well drill -stimulation-and post stimulation drill out process is now complete. Crews are on site installing all the production equipment. A tank battery has been erected consisting of 8 steel tanks capable of holding 400 barrels of fluid each. All tanks are situated inside a polymer lined steel containment dike area. 6 wells are present on the pad and all 6 are complete. Once the production equipment has been installed flow back operations will begin.
1128552033 2017-11-01 Preliminary Restoration No Violations Preliminary Restoration status check inspection found the well site meets ORC 1509.072 with vegetation established in the affected area of the well pad construction disturbance and erosion control in place. The well pad entrance is gated with signage in place. The lease road and well pad is of a stone surface. Addressing the wells on the pad, the drilling process is now complete and the 6 wells on the pad are awaiting stimulation with night caps on the well heads. Gauges are in place to monitor annular pressures. The tin horn cellars have been filled in with river gravel with fluid recovery tubes set inside the cellars. Also on the well pad is the tank battery consisting of 8 steel construction 400 barrel capacity tanks that are situated inside a polymer lined steel containment dike area. The well pad perimeter is constructed of an earthen berm with 3 catch basin fluid discharge points with the drain valves in the closed position. Contract workers from Hall Drilling are on site erecting the sales line and vapor recovery system burners.
1243739435 2018-05-02 Completion Testing No Violations Status check inspection found the well drilling process is now complete and crews from lighting energy are on site opening the toe sleeve valves and pressure testing the 5.5 diameter production casing string to 11,500 psi. The completion well head system is company identified as Downing and the well head consists of a manually operated lower master valve and the upper hydraulic master valve followed by a flow cross section. A support equipment is placed on polymer containment. Stimulation is slated begin in the next 14 days.
1421569806 2018-07-12 Completion Testing No Violations Status check inspection found the well stimulation process complete with a total of 65 stages complete. US Well is on location completing the last 2 wells on the pad out of 6 total wells. Annular pressure monitoring is in place on the 9.625 and 13.375 well head sections.
1494005754 2017-05-05 Casing String Witnessed No Violations On site inspection to witness the surface casing cement job. Patterson Drilling rig 342 is on location along with Schulumberger Cement Service and Wyoming Casing Service. Patterson Drilling has drilled a 17.5 diameter casing string to a measured depth of 1,250. Upon reaching total depth the drill string was pulled and 13.375 J-55 Grade buttress thread casing was ran in the well to a depth of 1,240. A casing shoe was ran along with a float collar was ran in the string at a depth of 1,195. Wyoming Casing Service assembled the string connections to a minimum torque of 13,000 foot pounds of torque. Schlumberger Cement Service cemented to casing using a 247 barrel cement mix consisting of 1165 sacks of class A cement with a 1.19 yield by design.. The cement weight was verified at 15.6 pound per gallon by weight of scale. Upon the landing of the casing string the rig established circulation to surface and Schlumberger pumped 30 barrels of gel spacer followed by the primary cement mix and finished the job by releasing the cement plug and displacing the cement to the float collar with 185 barrels of recycled fluid. Upon the landing of the cement plug the casing was pressure tested to 1019 psi with a final displacement pressure of 491 psi. Gel spacer returns were witnessed to surface at 95 barrels into displacement and cement returns were witnessed to surface at 125 barrels into a total of 191.6 barrels of total displacement.
1533564691 2018-08-06 Completion Testing No Violations Status check inspection on the well completion process found the stimulation process is now complete and a service rig owned by Fortis Well Service is on location and rigged up on the well. The rig is drilling the completion plugs out at a depth of 12,300 going to a total depth of 21,696. The snubbing unit is owned by QES company. The company is flaring gas and I reminded them of their permit condition requirement to notify the county inspector and local fire department when they are flaring. I also pointed out the close proximity of the flair stack to their processed fluid tanks. Also discussed with the company men on site was annular pressure monitoring and the removal of the outer well head B section valve by the service rig company without the installation of the VR isolation plug in the well head. The service rig and support equipment is situated on polymer containment. A second service rig is scheduled to arrive and begin completing another well on the pad in the next 48 hours.
