0085159621 2018-01-30 Casing String Witnessed No Violations On 01/28/2018 the rig drilled a 32-inch diameter borehole to a depth of 100 feet from ground level. Then, 100 feet of 26-inch diameter weld connection casing was inserted into the borehole. They cemented the casing with 67 barrels total volume of 322 sacks of Class A cement with a 1.18 yield, 2 percent calcium chloride, 0.5 pound of flake per sack, displaced with 56 barrels of fresh water, and had 11 barrels of returns at surface
0617674635 2019-02-04 Completion Testing No Violations On site inspection found the well drilling and stimulation process is now complete. The well was stimulated in 83 stages with isolation plugs set in the 5.5 diameter production casing at depths of 8,840 and 8,750. A tubing snubbing unit owned by Deep Well Services was on location and rigging up on the said well. The snubbing unit identified as unit # 6 will drill out the completion plugs on all 6 wells on the pad. The completion well control blow out preventer is owned by KLX Energy. The pressure pumping service is will be provided by Key Energy with a Sand X system for filtration of the well bore materials returned to surface. The completion equipment is situated on polymer containment with poly mats placed on the containment. The well pad perimeter drain valves are in the closed position. Production equipment is present on the pad. Also noted on this day I spoke with the Antero Company Representative on site Bryan Carter and we also called a Doug Phenix regarding the absence of annular pressure monitoring on the well head A&B sections. The men were informed of ORC 1501-9-1-08 section (5) of the well construction rules regarding annular pressure monitoring.

The rig is currentlyreached a total drilling depth in the 8.5 production well bore at a measured depth of 18,593and is pulling the drill bit at 9,168 for a directional drilling motor change. The total drilling depth proposed it 25,400.

1529412912 2018-02-20 Drill / Deepen / Reopen No Violations Status check inspection found H&P Drilling Rig 519 on location and preparing to begin drilling the 12.250 diameter intermediate well bore section. The Blow Out Preventer has been installed on top of the B section of the well head sub base and consists of a manual master valve with a flow cross assembly on top of the master valve, 2 choke valves are on each side of the flow cross section outlet lines on being manual on being hydraulic. Situated above the flow cross section is 2 sets of pipe rams with a set of blind rams in between the pipe rams. The 13.375 diameter casing string was set on 02/19/2017 and not witnessed due to the holiday. The 13.375 diameter casing was set to a depth of 1,222 and the cement report indicated 58 barrels of cement returns to surface.
