0524965873 2019-03-11 Drill / Deepen / Reopen No Violations On this day, I am conducting a follow-up inspection to report the annular pressures. The 9.625 inch diameter intermediate casing is currently at 2,100 pounds per square inch, and 13.375 inch diameter surface casing is at 0 psi. I spoke with Gulfport Representative Josh Starcher on 3/9/19 and 3/10/19. Pro-Flow was onsite for those days and had flowlines connected to a gas buster tank. The 9.625 inch diameter intermediate casing was blown down to 0 psi from 2,300 psi on 3/08/19, returning 0 barrels of fluid. The pressure recovered to 1,500 psi on 3/9/19, and blown down to 0 psi, returning 0 barrels of fluid. The pressure recovered to 1,500 psi on 3/10/19, and blown down to 0 psi, returning 0 barrels of fluid. It will be noted that Mr. Starcher stated that when blowing down the well, there is no volume to the gas, and blows down to 0 psi within 1-2 seconds.

The current pressure reading on the 9.625 inch diameter casing is 2,400 pounds per square inch, and the pressure on the 13.375 inch diameter surface casing is 0 psi.
0895122539 2017-11-01 Preliminary Restoration No Violations Preliminary Restoration status check inspection found the area affected by the well pad construction has vegetation established and erosion control in place. The lease road and well pad is of stone construction with the well pad entrance gated. Signage is present at the lease road entrance off of State Route 26. There are 6 wells on the pad that has been drilled but not completed. No production equipment has been placed on the well pad at the time of my inspection.

1500547867 2017-07-20 Casing String Witnessed No Violations On site inspection to witness the surface casing cement job with Pioneer Drilling rig 73 set up on the well and O-Tex cement service on location to pump the cement. The rig has drilled a 17.5 surface borehole to a measured depth of 1,054 from a drilling rig Kelly bushing heigth of 28.5. Upon reaching total depth the rig then inserted 1,054 of 13.375 diameter 54.5 pound per foot J-55 Grade Buttress thread casing with a guide shoe and float collar casing equipment ran along with 7 casing centralizers. The last casing joint was circulated into the well bore and the hole circulated until the arrival of the Cement company. O-Tex cement service cemented the casing by first pumping a 50 barrel dyed gel spacer followed by a 156 barrel cement mix consisting of 960 sacks of class H cement. 30% cement excess was ran. The cement was displaced with 156 barrels of fresh water.32 Barrels of cement returns were witnessed to surface and the casing was pressure tested to 1010 psi or 500 psi over displacement pressure upon the landing of the plug.

1502390252 2017-08-10 Drill / Deepen / Reopen No Violations Status check inspection on the well drilling process found Pioneer Drilling rig 73 on location and currently drilling the 8.5 diameter production borehole at a measured depth of 8,570 and just beginning to drill the horizontal cureve section. The proposed total depth to be drilled is 18,592. The drilling rig and support equipment is placed on polymer containment.
1916730531 2019-03-08 Drill / Deepen / Reopen No Violations I am onsite to witness Pro-Flow bleed down the pressure of the 9.625 inch diameter intermediate casing. They are currently connecting the iron to the gas buster and will pressure test the lines prior to flowing into the tank. I spoke with Gulfport Representative Josh Starcher onsite and informed him to monitor the pressures, and update ODNR with the new pressures after 24 hours.

2116763305 2019-03-06 Drill / Deepen / Reopen No Violations On this day, I found the pad gated with signage at the entrance. There are (6) wells on the pad that have been drilled, but not completed. Steel fencing surrounds the perimeter of the wellheads. Each well has annular pressure monitoring devices equipped. Also noted, each well has signage stating that a back pressure valve (BPV) is installed. The 9.625 inch diameter intermediate casing has a pressure of 2,750 pounds per square inch (psi), and the 13.375 inch diameter surface casing has a pressure of 0 psi. I contacted Gulfport Energy Production Superintendent Justin Hoague to inform him of the pressures. Gulfport Representative Josh Starcher arrived onsite, and he made contact with additional Gulfport personnel. Also, Mr. Starcher temporarily bled the pressure down to 1,000 psi within several seconds. Pro-flow will be onsite within the next 48 hours to bleed the pressure down into a gas-buster tank. I informed Mr. Starcher to inform me of any updates. He also noted that the wells were recently equipped with pressure gauges. It was indicated that the wells may be completed in 2020.
