0558138116 2019-01-07 Drill / Deepen / Reopen No Violations On this day, I found the Patterson 714 rig currently testing the BOPs and conducting maintenance. I spoke with Duane Goodsell and Joe Wunderlich during the inspection. The 17.5 inch diameter production borehole was drilled to a depth of 1,717 feet, and the 13.375 inch diameter surface casing was set at a depth of 1,693 feet. C&J Energy Services cemented the casing back to surface, with 25 barrels of returns. Drilling of the 12.25 inch diameter intermediate borehole is projected to commence after the rig repairs are completed.
1182295680 2019-01-20 Casing String Witnessed No Violations On site inspection at 17:45 hours to witness the surface casign cement job. The rig on location and rigged up on the hole is Patterson Driling rig 714, the cement company is C&J and the casign crew is TRK. The rig has drilled a 17.5 diameter well bore to a measured depth of 2000 with the depth measured off a drilling fig floor heigth of 26 above the well head flange. The rig inserted 1,977 of J-55 grade Buttress Thread casign with a casing guide shoe and float collar ran and proper centralizer placement demonstrated. C&J Cement service will cement the casing using a 305 barrel cement mix consisting of 1,650 sacks of class A cement. Due to time restraints with the National Holiday I was unable to witness the cement job. All cement calculatins along with the casing tally was confirmed.
1349275115 2019-05-24 Drill / Deepen / Reopen No Violations On this day, I found the pad fenced and gated with signage at the entrance. An earthen berm surrounds the perimeter of the pad. The drilling process is finalized, and the well is not completed. A nightcap is currently in place. The timeframe is unknown as to when the completion process will commence. It will be noted that ProPipeline is currently working on the northwest slope of the pad, and have removed a section of the fencing and berm. ODNR Engineering Technician Andrew Thomas will be notified. The preliminary restoration will be addressed at a later date.
1495202114 2017-05-19 Casing String Witnessed No Violations On site inspection to witness the conductor casing cement job.with Bryan Constructon top hole rig and O-Tex cement surface. The rig has drilled a 36 diameter conductor borehole to solid bed rock at 94 and set 26 .375 wall casing to 94 measured depth from ground level. O-Tex cement service cemented the casing using a 81.9 barrel class H cement mix with the cement weight being verified at 15.6 pound per gallon by weight of cement. 26 barrels of cement returns were witnessed to surface.
1545411903 2018-12-21 Drill / Deepen / Reopen No Violations On this day, I found the pad construction remediation process continuing. I spoke with Company Man Duane Goodsell, and he indicated that it will take several days to complete the rig move due to the one-lane road. Some minor equipment and trailers are being placed on the pad, but the Patterson 714 rig has yet to be transported from the previous pad. At this time, they are unsure as to which permit number will be drilled first.
1551798887 2019-03-05 Drill / Deepen / Reopen No Violations On this day, I am conducting an inspection in response to an incident reported to the ODNR OneCall on 03/04/2019 pertaining to approximately 5 barrels of oil-based drilling fluid being released onto the pad from a defect in the Kelly hose while drilling. Remediation commenced immediately, and have conducted surficial scraping of the affected area on the pad (primarily around the catwalk) and placed the contaminated product within a plastic lined roll-off for proper disposal. Due to the low temperatures, the pad was frozen and allowed for them to suck and scrape most of the product. They are continuing to utilize vac trucks to remove any additional product from the impacted containment liner and replace it with new liner. I spoke to EdgeMarc Safety Representative Ben during the inspection and he stated that a new Kelly hose has been installed and pressure tested. The Patterson 714 rig is currently drilling the 8.5 inch diameter production borehole at a measured depth of 18,032 feet. The most recent survey at 18,007 feet had an azimuth of 337.22 degrees and an inclination of 90.80 degrees.

1961811620 2018-12-12 Drill / Deepen / Reopen No Violations On this day, I spoke with Company Man Duane Goodsell on location and he indicated that Had Drilling drilled a 19 inch diameter mouse hole within the cellar, and ran 80 feet of 16 inch diameter casing with a welded steel plate on the bottom. He stated that the mouse hole will be grouted on 12/13/2018. It will be noted that an existing mouse hole within the cellar will be abandoned and cemented back to surface. Additionally, pad construction is being conducted to remediate a slip issue prior to the Patterson 714 rig drilling the planned wells.
