1331864564 2018-04-17 Drill / Deepen / Reopen No Violations Status check inspection on the current drilling process found H&P Drilling rig 519 on location and currently conducting a formation integrity test on the open hole formation just under the 9.625 casing string set in the Reedsville at a measured depth of 7,972. They are testing with 12.5 pound per gallon oil based drilling mud to an equivalent of 14.6 pound per gallon.
1386282098 2018-04-24 Drill / Deepen / Reopen No Violations Status check inspection to report the well drilling process is now complete. The rig on location H&P 519 has drilled the 8.5 diameter production borehole to a measured depth of 17,525 and set 5.5 diameter production casing to a depth of 17,510, The top of the cement on the 5.5 production casing was 6,972 by design. The rig has skidded off the well and a night cap has been installed
1517243354 2018-01-29 Casing String Witnessed No Violations On site inspection to witness the conductor casing cement job accompanied by ODNR Inspector Tom Thomas. Express Energys top hole rig is on location and spudded the said well on 01/28/2018. The rig has drilled a 32 diameter conductor borehole to a measured depth of 100 from ground level. Express Energy cement service cemented the casing using a 67 barrel absolute volume cement mix consisting of 322 sacks of class A cement mix with 2% calcium and .50 pound per sack loss circulation material. The cement was displaced to 90 inside the casing with no cement returns witnessed to surface inside the cellar. A Redi mix cement truck was brought in to top off the said well along with other wells on the pad cemented on this day with 10 cubic yard of cement applied to the well from surface and cement witnessed to surface. Inspector Thomas will enter the casing ticket. Cement weight was verified by weight of scale.
1517244394 2018-01-29 Casing String Witnessed No Violations On this day, I am accompanied by ODNR Inspector William Stolarik. Onsite inspection to witness the conductor casing cement job with Express Energy Services. The well was spudded on 01/28/18. The rig drilled a 32-inch diameter borehole to a depth of 100 feet from ground level. Then, 100 feet of 26-inch diameter weld connection casing was inserted into the borehole. They cemented the casing with 67 barrels total volume of 322 sacks of Class A cement with a 1.18 yield, 2 percent calcium chloride, 0.5 pound of flake per sack, and displaced with 56 barrels of fresh water. No cement returns at surface. Cement level is approximately 40 foot down hole. Caldwell Redi-mix grouted with 10 cubic yards down the annulus and back up to the cellar. Cement returns witnessed at surface.
1523259916 2018-04-09 Casing String Witnessed No Violations On site inspection to witness the surface casing cement job with H&P Drilling rig 519 on location along with Schlumber Cement Service. The company representatives are Carlos Argos and Brandon Jeffcoat. The rig has drilled a 17.5 diameter well bore to a measured depth of 1,176 off a drilling rig Kelly Bushing heigth of 30 above ground level. The rig ran 1,176 of 13.375 diameter J-55 Grade buttress thread casing with a guide shoe and float collar ran. Schlumberger Cement Service cemented the casing by first pumping well bore capacity of fresh water followed by a 30 barrel 6% gel pacer then a 231 barrel cement mix consisting of 1,088 sacks of class A cement. The cement was displaced to the float collar with 175 barrels of fresh water with 25 barres of cement returns witnessed to surface. The final displacement pressure was 505 psi and the casing test pressure was 1,180 psi. The casing float collar held. The well head (A) section is a Cameron 10,000 psi rated and the casing was set in a mandrill hanger.
1535546496 2018-08-29 Preliminary Restoration No Violations Preliminary Restoration status check inspection found the well site meets ORC 1509.072 with erosion control in place and vegetation established. The lease road and well pad surface are of stone surface construction and in good repair. Signage is posted at the entrance of the lease road. There are 6 wells drilled on the pad. The said well has been drilled and is awaiting stimulation. There is a night cap on the B section of the well head. The company does have gauges on the A&B section valves as well as the night cap. Crews are on location install the production tanks and flow lines. The production tank farm is built and consists of (10) steel construction 400 barrel capacity tanks that are situated inside a polymer lined steel containment dike area. The well pad perimeter is protected by an earthen berm with multiple sumps and drain valves situated on each side of the well pad.
1549321924 2019-02-04 Completion Testing No Violations On site inspection found the well drilling and stimulation process is now complete. The well was stimulated in 43 stages with isolation plugs set in the 5.5 diameter production casing at depths of 8,830 and 8,740. A tubing snubbing unit owned by Deep Well Services was on location and rigging up on the Loraine #1-H. The snubbing unit identified as unit # 6 will drill out the completion plugs on all 6 wells on the pad. The completion well control blow out preventer is owned by KLX Energy. The pressure pumping service is will be provided by Key Energy with a Sand X system for filtration of the well bore materials returned to surface. The completion equipment is situated on polymer containment with poly mats placed on the containment. The well pad perimeter drain valves are in the closed position. Production equipment is present on the pad. Also noted on this day I spoke with the Antero Company Representative on site Bryan Carter and we also called a Doug Phenix regarding the absence of annular pressure monitoring on the well head A&B sections. The men were informed of ORC 1501-9-1-08 section (5) of the well construction rules regarding annular pressure monitoring.
