1099729351 2018-09-24 Completion Testing No Violations Status update inspection found a total of 10 wells present on this well pad. All 10 have been drilled and not completed. Each well is equipped with a lower A and upper well head section with 2 diameter riser pipes present on each section with valves and gauges present on each section for annular pressure monitoring. The casing head does have a multi valve set up with a mechanical gauge present. No tubing is in the well. No production equipment is present on the well pad other than the 3rd party mid stream pipeline. The well pad does have an earthen berm in place. The well pad surface and lease road is of stone construction and in good repair. Addressing the preliminary restoration. There is an area along the lease road that is within the well site boundary that was recently disturbed by the installation of a transportation pipeline. There is also a area on the West side of the well pad that was disturbed and vegetation not established. Larger diameter wooden poles are set on the East side of the well pad along the pad surface boundary for sound barrier during stimulation. There are (2) 12 diameter trunk lines that have been laid along the well pad perimeter, these lines are fresh water lines to support the stimulation process. The lease road does have a gate present.
1212976502 2017-10-19 Casing String Witnessed No Violations On this day, I accompanied ODNR Inspector William Stolarik. Please refer to his comments for a detailed report. On this day we are on-site to witness the surface string being cemented. Pioneer rig 73 had drilled to a total depth of 1390 feet and Stingray Pressure Pumping is on location to cement the surface casing. Witnessed cement returns to surface.
1508406730 2017-10-19 Casing String Witnessed No Violations On site inspection to witness the surface casing cement job accompanied by ODNR Inspector Tom Thomas. Pioneer Drilling rig # 73 is on location and rigged up on the hole. The rig has drilled a 17.5 diameter surface borehole to a measured depth of 1,390 off of a drilling rig Kelly Bushing heigth of 28.5. The rig pulled the drill string after making 3 soap sweeps to condition the well bore and then inserted 1,390 of 13.375 diameter buttress thread J-55 grade casing with a guide shoe and float collar casing equipment ran along with 9 casing centralizers. Circulation was established with the drilling rig while landing the last 2 casing joints. Upon landing of the casing 2 bottoms up circulation cycles were pumped before turning the job over to Stingray who cemented the casing by first installing the cement head and then pumping 25 barrels of 6% gel spacer followed by a 263 barrel cement slurry consisting of 1,251 sacks of class A cement. The cement was displaced with 207 barrels of fresh water to the float collar with the casing being pressure tested to 1,175 psi with a final displacement pressure of 606 psi. Cement returns were witnessed at 180 barrels into displacement.
1512488185 2017-12-05 Production Wells No Violations Status check inspection to document the well completion status. Pioneer Drilling rig 73 drilled the 8.5 diameter production borehole to a measured depth of 18,124 measured off a drilling rig Kelly busing heigth of 28.6 above ground level. The rig pulled the drill string and inserted 5.5 diameter production casing and cemented it on 11/19/2017. The cement displacement was 400 barrels and upon reaching total displacement the cement plug did not land with a final displacement pressure of 3,300 psi, the cement company checked to see if the string floats were isolated and the well flowed back 6 barrels of fluid indicating the plug was not latched and the floats were not holding. At that point the well was shut in with 3,300 psi on the casing and the cement allowed to cure before setting a isolation bridge plug inside the 5.5 diameter casing to allow the well head blow out preventer to be removed and the night cap put on the well.
1665651512 2019-05-16 Completion Testing No Violations On this day, I found the drilling and hydraulic fracturing processes are completed. The frac plugs have been drilled out and the well is currently in the flowback stage. I spoke with Gulfport Representatives Corey Cloud and Robert Coots during the inspection. Annular pressure monitoring devices are in place and pressures are logged daily. Identification is posted at the wellhead.

1841242938 2018-06-18 Completion Testing No Violations On this day, I am conducting an inspection per request of ODNR Inspector William Stolarik. I spoke with Company Man, Ken Miller, during my inspection. (10) wells were drilled on the pad. They are currently plumbing the intermediate and surface casing, at the wellhead, and preparing to install pressure monitoring devices. The grates will be replaced over the cellar, when they are finished.
2145687492 2017-10-23 Drill / Deepen / Reopen No Violations Status check inspection on the well drilling progress found Pioneer Drilling rig # 73 on location. The rig has drilled the 12.250 intermediate well bore to a measured depth of 8,088 and is pulling the drill string out of the hole to change out a failed drill bit. The drilling rig and support equipment is situated on polymer containment and the well pad earthen berm is serving as the primary containment with the drain valves in the closed position. The rig has approximately 700 to drill before reaching total depth for this section.
