0009929204 2018-02-04 Casing String Witnessed No Violations On site inspection to witness the surface casing cement job with Patterson Drilling rig 583 on location along with Stingray Cement Service and Wyoming Casing. The rig has drilled a 17.5 diameter well bore to a measured depth of 1,574 with the depth measured off a drilling rig Kelly Bushing heigth of 26 above ground level elevation. Upon reaching total depth the rig pumped several soap sweeps before loading the hole with fresh water and pulling the drill string. Once the drill string was out of the hole the rig inserted the 13.375 diameter J-55 Grade Buttress Thread casing with a 44.37 casing shoe track ran consisting of a casing float shoe and collar and one casing joint. The casing was loaded with fresh water and the last casing joint circulated into the well bore before hooking up the cement head and commencing the cement job by first pumping 25 barrels of 6% gel spacer followed by a 345 barrel absolute volume cement mix consisting of 1,620 sacks of Class A cement. The cement yield was 1.19 and weight was 15.6 pound per gallon with the cement weight verified by weight of scale. The cement was displaced to the float collar with 252 barrels of fresh water and the final displacement pressure was 683 psi at 3.18 barrel per minute. The plug landed at 252 barrels total displacement and the casing was pressure tested to 1,197 psi. Casing floats held and 62 barrels cement returns witnessed to surface.
1418995337 2017-10-19 Casing String Witnessed No Violations The purpose of my inspection was to witness 110 feet of the 26 inch diameter conductor being cemented in a 36 inch diameter borehole 110 feet in depth. I witnessed Lightning Energy run the 26 inch diameter conductor and Stingray Cementing rig up on location. A pre-job safety meeting was held with all personnel on location including Gulfport Company Man, Stingray Cementing, Petta Enterprises, and Lightning Energy. Following the safety meeting Stingray Cementing pumped 15 barrel gel, 10 barrels of fresh water, 90 barrels of Class A cement, and displaced with 58 barrels of fresh water leaving 10 feet of cement fill within the 26 inch diameter conductor. Cement returns were witnessed in the cellar.
1518981384 2018-02-18 Drill / Deepen / Reopen No Violations On 02/16/2018 from the Company man on site indicating they had experienced a leak on the drilling rig stand pipe system that caused oil based drilling mud to be sprayed onto the location and perimeter berm. On they day of the incident I was in the presence of my direct Supervisor Brent Bear attending to another incident and we review the submitted photographs from the company and the proposed remediation verbal plan of recovery action to address the affected area. The company reported they estimate less than on barrel (42 US gallons) was discharged in a spray atomized form over a small area of the well pad. Upon inspection it was observed that the product had been recovered off the gravel area and new gravel put down and the area of the berm affected had a peat moss type absorbent material applied. 1559058315 2019-05-28 Production Wells No Violations On this day, I found the pad fenced and gated with signage at the entrance. An earthen berm surrounds the perimeter of the pad. There are (3) producing wells, (3) gas processing units (GPU) with sand traps, and (4) 400-barrel and (1) 50-barrel steel production tanks within polymer lined steel containment. An Emergency Shutdown (ESD) button is present at the production facility, wellheads, and entrance. Additionally, concrete barriers surround the perimeter of the wellheads and GPUs. Identification and annular pressure monitoring devices are present at the wellheads. The 9.625 inch diameter intermediate casing has a pressure of 180 pounds per square inch (psi), and the 13.375 inch diameter surface casing has a pressure of 50 psi.
1763369158 2018-08-28 Preliminary Restoration No Violations Preliminary Restoration status check inspection found the area affected by the well pad construction meets ORC 1509.072with erosion control and vegetation established. The lease road and well pad surface are of stone construction and in good repair. The well pad perimeter is protected by an earthen berm. Signage is posted at the lease road and well pad entrance with identification on each well head and hazard warning labels on the tank battery. The well head assembly consists of a lower A and upper B section with 2 diameter riser pipes attached to the outlet ports of both sections. The riser pipes are also equipped with valves and gauges to monitor annular pressures. The A section pressure which is the pressure between the 13.375 and 9.625 diameter casing strings shows 6 pounds pressure. The B section which is the pressure between the 5.5 and 9.625 casing strings shows 599 pounds pressure on the gauge. The 5.5 diameter casing string internal pressure is 5,489 pounds pressure and the production tubing is 4,937 pounds pressure. The 3 wells present on location are all in the flow back stages flowing through a sand trap and choke manifold. There are a total of (7) 500 barrel capacity portable tanks on location the wells are being flown into. The production fluid facility is on the back right hand corner of the well pad and consists of (4) steel construction 400 barrel capacity tanks along with a 50 barrel steel tank, all situated inside a polymer lined steel containment dike area. There is a EQT midstream operation on the left of the lease road as you enter the well pad.
