0294003523 2017-12-14 Drill / Deepen / Reopen No Violations Status check inspection on the well drilling progress found Orion Drilling Polaris rig # 14 has recently completed the drilling of the 12.250 intermediate section and ran 9.625 casing into the well bore, the casing was cemented in place with no reported issues. The rig has skidded over to the next well on the pad the Islay 1-H to drill the 17.5 surface hole. Once all the vertical sections are complete on the 3 wells on the pad the rig will drill the 8.5 diameter production boreholes.
1280447008 2018-08-15 Completion Testing No Violations Status check inspection on the well completion process found Halliburton Pressure pumping service on location stimulating the 3 wells present and Horizontal Wireline on location providing the wireline service. The last stage treated was stage 28 with a average treating pressure of 10,000 pounds and break pressure of 8,400 pounds per square inch pressure (psi) the initial shut in pressure for the wells on the pad has been averaging 6,400 psi. Horizontal wirelline perforated the stage in 6 clusters with 40 perforations total. The stage was perforated between the depths of 17,993 and 18,159 and a completion plug will be set at 18,173. The company is monitoring all annuli electronically. All stimulation and support equipment is placed on polymer containment.
1512514404 2017-12-05 Casing String Witnessed No Violations Status check inspection to witness the surface casing cement job with Orion Drilling Polaris rig 14, Schlumberger Cement Service and Franks International Casing Service. The rig has drilled a 17.5 diameter surface borehole to a measured depth of 1,990 off a drilling rig Kelly Bushing heigth of 26 above grade level. The hole was drilled on fluid for the first 600 and then finished on air with the company pumping 3 soap sweeps upon reaching total depth then loading the hole with fresh water and pumping a circulation cycle. After cleaning up the hole the drill string was pulled and the 13.375 diameter J-55 grade buttress thread casing was inserted into the well bore with a casing guide shoe ran along with a float collar completing the 42.65 casing shoe track. Also noted was 24 casing centralizers were inserted on the casing providing proper casing stand off from the well bore. The casing was cemented by Schlumberger Cement Service using a 365 barrel absolute volume cement mix consisting of 1,703 sacks of class A cement. The cement was pumped by Schlumberger and displaced by the rig pump with 80 barrels of cement returns being witnessed to surface and the casing being pressure tested to 2,100 pounds per square inch upon the landing of the plug by Schlumberger.

1945703014 2018-07-30 Completion Testing No Violations Status check inspection on the well completion process accompanied by ODNR Inspector Tom Thomas along with Darrin Galisky representing Stat Oil. Current operations are the well drilling process is complete and Halliburton pressure pumping along with Allied Horizontal Wireline is rigging up preparing to begin stimulation operations on the 3 wells present on the pad. All stimulation support equipment is situated on polymer liner. The well head completion stack consists of a primary master valve situated atop the B section of the well head followed by a hydraulic master valve then the well head flow cross. Atop the flow cross is a upper hydraulic master valve followed by the pressure lines before the wireline head attachment point. There is a separate accumulator control for each well on the pad. The well completion operations are to begin in the next 48 hours.
1957701366 2018-07-30 Completion Testing No Violations On this day, I accompanied ODNR Inspector William Stolarik. Please reference his comments for a detailed report. Halliburton is in their preparation phase to begin hydraulic fracturing. We spoke with Statoil Field Completions Superintendent, Darren Galisky, and conducted a walkthrough of the pad and equipment onsite.
2011564395 2019-04-16 Preliminary Restoration No Violations On this day, I am conducting a preliminary restoration inspection. The site meets ORC 1509.072 (A). I found the pad gated with signage at the entrance. There are (3) producing wells, (3) gas processing units, (3) 400-barrel steel production tanks within steel containment, and an Emergency Shutdown (ESD) button is present at the pad entrance. Concrete barriers surround the perimeter of the wellheads. Each well has annular pressure monitoring devices equipped. Greg Davis, Equinor Representative, was notified to label the gauges and wellheads with proper identification. Mr. Davis also stated that annular pressures are monitored daily. It will be noted that a third-party contractor, FMC Technologies, is onsite with secondary sand traps.
