0687571042 2018-02-18 Drill / Deepen / Reopen No Violations On this day, I accompanied ODNR William Stolarik. Please refer to his comments for a detailed report. The Patterson 803 rig is currently at a depth of 7,160 feet drilling the 12.25 inch diameter borehole at an inclination of 29.32 degrees and an azimuth of 256.35 degrees.

1251869608 2018-02-12 Drill / Deepen / Reopen No Violations During my inspection Patterson drilling rig 803 was making up the bottom hole assembly preparing to spud the 17.5 inch diameter borehole. At this point the well was constructed with 26 inch diameter conductor and 18.625 inch diameter surface casing. While drilling the 24 inch diameter borehole in the surface section a coal seam was encountered from 843 feet to 857 feet in depth.
1518603799 2018-02-14 Casing String Witnessed No Violations On this day, I accompanied ODNR Inspector William Stolarik to witness the 13.375-inch diameter surface casing cement job with Halliburton Energy Services. The rig drilled a 17.5-inch diameter borehole to a depth of 2,508 feet from a kelly bushing of 28 feet. Then, Mikes Power Tong Services inserted 2,481 feet of 13.375-inch diameter J-55 54.50 lb/ft casing into the borehole. They cemented the casing with 50 barrels of 6% gel spacer, 321 barrels total volume of cement consisting of 1,465 sacks of Class A cement with a 1.23 yield and a weight of 15.8 pounds per gallon. It was displaced to the float collar with 382 barrels of 9.6 pounds per gallon brine water. The plug landed with 1,000 psi and then pressure tested at 1,577 psi. 25 barrels of cement returns were brought to surface.
1518604852 2018-02-14 Casing String Witnessed No Violations On site inspection to witness the surface casing cement job accompanied by ODNR Inspector Tom Thomas. On this day it was observed that Patterson Drilling rig 803 was on location along with Halliburton Cement Service and Mikes power tong service. The rig has drilled a 17.5 diameter 1st intermediate well bore to a depth of 2,508 measured off a drilling rig Kelly Bushing heigth of 28 above ground level elevation. Upon reaching total depth the well bore was loaded with 9.6 pound per gallon brine before puling the drill string. Once the drill pipe was out of the hole the rig inserted 2,4581 of 54.5 pound per foot J-55 grade buttress thread casing with a guide shoe and float collar ran and centralizers every 3rd casing joint collar. Halliburton Cement Service cemented the casing by first pumping 50 barrels of 6% gel spacer followed by a 321 barrel absolute volume cement mix consisting of 1,465 sacks of class A cement with a cement weight of 15.8 pound per gallon and yield of 1.23. The cement weight was verified by weight of scale. The cement was displaced to the float collar with 382 barrels of recycled bring water with the rig pumps and pressure tested to 1,577 psi upon the landing of the plug with 25 barrels of cement returns witnessed to surface.
1776171104 2019-04-08 Production Wells No Violations On this day, I am accompanying ODNR Engineers Mark Atkinson and Andy Thomas, ODNR Southeast Supervisors Brent Bear and Mike Moreland, and CNX Gas Representatives John Hardcastle, Rick Harless, and others. The meeting is to review the new proposed well locations. The existing production wells were also inspected. The 9.625 inch diameter intermediate casing has a pressure of 900 pounds per square inch (PSI), and the 13.375 inch diameter surface casing has a pressure of 670 PSI. On 4/8/2019 I left a message with John Zachwieja, CNX Production Foreman, to notify him of the pressures.
1979206641 2019-01-22 Preliminary Restoration No Violations On this day, I am conducting a preliminary restoration inspection. Site meets 1509.072 (A) Vegetation is established with no substantial erosion. I found the pad gated with signage at the entrance. There are (4) producing wells, (4) gas processing units (GPU), (4) 400-barrel and (1) 100-barrel steel production tanks within polymer lined steel containment, and an Emergency Shutdown (ESD) button is present at the pad entrance. Additionally, concrete barriers surround the perimeter of the wellheads and the GPUs. Each well has identification and annular pressure monitoring devices equipped. The 9.625 inch diameter intermediate casing has a pressure of 900 pounds per square inch (psi), and the 13.375 inch diameter surface casing has a pressure of 380 psi.
