1369781327 2019-06-06 Production Wells No Violations On this day, during a site inspection for permit number 2-4869, the said well was located within the tree line approximately 65 from the potential new well location. I found the well equipped with 5.5 inch diameter casing, tubing, and rods inserted. The tubing is open to atmosphere and the casing is connected to a corroded 2 inch diameter steel flowline. It is setup to be produced by means of a shackle line pumping unit. The well does not show signs of recent activity and is surrounded by vegetation. No production equipment is associated with the well, and no identification is present. Multiple pictures were taken. I spoke to Dean Christman, and he indicated that he is not knowledgeable about the status of the well.

1560264263 2019-06-11 Production Wells No Violations On this day, I am conducting an inspection to verify the coordinates and spacing between the potential drilling location of permit number 2-4869 and 6-1058. The site is being built and the new well is not currently spudded. On 6/11/2019, Kerry Brown of Monroe Drilling had been notified about the concern. I also contacted ODNR Permitting Geologist Aaron Evelsizor. I spoke with Mr. Dean Christman (DC Oil & Gas) in regard to the spacing of API 34-111-6-1058 (Howell Sam #2) and the location of API 34-111-2-4869 (McCoy #1). The wells are located approximately 65 feet apart. I informed him that he would need to contact ODNR permitting, prior to commencing drilling the new well, to discuss his options for spacing.
