0386034325 2013-05-16 Administrative Inspection No Violations On this day I received a call from a attorney asking about the production records, After a brief conversation the attorney indicated a inspection or complaint would not be necessary. I will note that David Beverage has passed away and His widow is working on selling His producing wells.
0921090775 2015-01-06 Production Wells Violations Noted Inspection per request of the landowner Jerry Piatt as part of complaint #4900. Inspection of the well head found the following equipment present-6.625 casing-2 production tubing and rods with a 1 vent packer string present and the well set up to be produced by the means of a pumping unit. All valves are closed at the well head and the electric service is disconnected and laying on the ground. A single 100 barrel and a 60 barrel production tank are remotely located at the well site with no identification present. No gas metering equipment was found in the area related to the said well. Comm ercial gas was reported in 2010-2011& 2012 and the landowner claims he has received no royalty payments. Instructions are as follows-Plug the said well within 180 days. 1285904554 2015-12-15 Plug / Plug Back No Violations Follow up inspection to complaint #4900 and compliance notice 1420563120 found that a service rig owned by Chuck Dye is now on the hole and has pulled all downhole equipment from the well. A plugging pit is to be constructed and the well is slated to be plugged and a plugging permit has been issued.
1433767117 2015-06-08 Production Wells No Violations Follow up inspection to complaint #4900 and compliance notice 1420563120. Inspection of the well head found the following equipment present-6.625 casing-2 production tubing and rods with a 1 vent packer string present and the well set up to be produced by the means of a pumping unit. Contact was made with the well owner Patricia Beverage who indicated she was waiting on Chuck Dye to bring his service rig to plug the well, I informed Ms. Beverage that the Division has received no application to plug the well or form 7 transfer between Her and the landowner. Ms. Beverage indicated She would contact the Divisions Permitting section.
1450455203 2015-12-18 Plug / Plug Back No Violations On site inspection to witness the well plugging process. On this day I was accompanied by Inspector Jim Nicholson with the following observed. A service rig owned by Chuck Dye was on location and has pulled the production tubing out of the well and ran 2.375 in the well to a depth of 1,346, the well was logged on 12/17/15 by JET Wireline and the TD recorded at 1,415 with the 6.250 total depth found to be 1,337. Formation Cementing is on location and spotted a 101 sack bottom plug at 1,346 the rig then pulled 6 joints up to 1,166 and pumped a additional 50 sack plug in which the cement contractors saw 350 psi lift pressure. The rig then pulled the tubing up to 700 and pumped a gel spacer, at that point tubing was pulled to 300 and a surface plug set with good cement returns to surface and the cement stayed up to surface. The well did kick off during the setting of the initial bottom plug and sprayed oil onto the location outside the plugging pit.
1450613440 2015-12-19 Plug / Plug Back No Violations On site inspection to witness the clean up process from the events that occurred on 12/18/2015 when the well kicked off and flowed oil out during the plugging process while the bottom plug was being pumped into the well bore. The service rig owner Chuck Dye was on location with 3 or his hired crew cleaning up the oil that was inside and outside the plugging pit. The pit will be closed and due to the excessive mud conditions and geographical location the area will have to remain unrestored until the dry season, the landowner is aware of the issue. The contractor did utilize a unmaintained township right of way to access the area under the landowners direction, the landowner was advised that the Township Trustees and adjoining landowners should have been advised to the intent to use the roadway and the Division including the local inspector had no voice in the matter or direction in the utilization of the roadway, all parties were advised that all affected area shall be restored.
1504109765 2017-08-30 Final Restoration No Violations Final Restoration status check inspection found the (2) production tanks and (2) old pumping units still on location. The well site was never finish graded and does have vegetation established. The final restoration is not complete.

1821425830 2015-12-16 Plug / Plug Back No Violations On site inspection to witness the plugging preparation work. On this day I was accompanied by MRI 2 Inspector Jim Nicholson. Upon inspection it was observed that a service rig owned by a Chuck Dye was now rigged up on the hole and has pulled the production tubing and pumping rods out of the well. The rig ran a bailer in the well and a total depth was measured by a wireline measurement tool and was noted to be 1,424 with a fluid level of 350 from surface. A plugging pit is being dug and the service rig will attempt to bail the well dry with the intention of grouting the cement from surface using a 9 bag cement mix, this well was drilled to the Keener Sand formation by the means of a cable tool.
1892033819 2017-08-30 Final Restoration No Violations On the day of inspection I accompanied ODNR Inspector William Stolarik. Please refer to his comments for a complete report. We found (2) productions tanks and (2) old pumping units on location. Vegetation is established, but the area was never final graded. Final restoration is incomplete.
1933374008 2015-12-16 Plug / Plug Back No Violations Inspected well today with ODNR MRI-3 Bill Stolarik. Plugging rig has moved onto site and pit is being excavated. Observed rig to estimate depth of well at 1,424 feet by measurement of wireline. Tubing rods have been removed from well and moved off site. Operator will attempt to bail well dry.
