0316053620 2018-01-22 Production Wells No Violations Inspection follow up to complaint # 5035 accompanied by Inspector Tom Thomas. Upon inspection the well was observed to be equipped with 10 steel conductor and 5.187 diameter production casing and 2 diameter production tubing. The well is was equipped to be produced by the means of a shackle line pumping unit and pumping rods are present and inserted into the well bore, however the old underpull pumping unit has been removed and is laying on the ground. There is no production flow line attached to the casing or tubing well heads. The 2 tubing is broken off and is standing open. At the time of my last inspection on 05/27/2015 I contacted a local producer identified as Dean Christman who identified himself as the lease holder and well owner and asked him to remove the old production facility which consisted of multiple tanks located directly below the well that had well bore product in them. It was observed on this day that the tanks have been removed. I also requested Mr. Christman register the well, however the well is not yet registered. Photographs taken on this day.
0598084906 2015-05-27 Production Wells No Violations Status check inspection as part of complaint # 5035. Upon inspection the following equipment present at the well head 6.625 casing 2 tubing and pumping rods. With the tubing rusted into two above the well head and no flow lines or valves present on the well head. 2 steel production tanks were found present adjacent to the well next to a stream bed with one tank being of 100 barrels in capacity and empty and the other tank being of 20 barrels in capacity and being full of fluid. ODNR Idle Orphan Inspector Cindy Vandyke was contacted in regards to the full tank of fluid needing emptied and the well ownership of unknown status. It was suggested we address the issue of ownership to determine a course of action to empty the tank before it leaks into the stream. Dean Christman is the lease producer but does not claim to own the well and was contacted on this day in regards to the 3 non producing wells on the property at 740-310-1654.
0627247688 2018-01-22 Production Wells No Violations On this day, I accompanied ODNR Inspector William Stolarik to conduct a follow-up inspection to complaint 5035. Please reference his comments for a detailed report. The well is equipped with wooden conductor, 10 inch diameter steel casing, 5.1875 inch diameter production casing, and 2 inch diameter tubing with the pumping rods still attached.
