1423263038 2018-01-19 Production Wells No Violations Status check inspection as a follow up to complaint # 5035 filed by the landowner Ronald Davis on 05/27/2015. Upon inspection the well was observed to be located directly behind the residence of Mr. Davis on 42323 Plainview Road Woodsfield Ohio. The well was observed to be equipped with 6.625 diameter production casing and 2 diameter production tubing. There is no flowlines attached to the casing or tubing well heads. The well does have a pumping unit attached to it but is in obvious sign of disrepair and not capable of operating. Mr. Davis indicated that the electric service was removed from the well in or around the year 2002. There is no production meter or tank located in the area that can be identified as associated with the well. The landowner has indicated that D-C Oil was the last producer of the said wells along with additional wells on the property all in various state of disrepair. Photographs taken on this day.
1432744866 2015-05-27 Production Wells No Violations Status check inspection as a response to complaint # 5035 filed by the landowner Ronald Davis. Upon inspection of the well head the following equipment was found present-6.625 casing-2 production tubing and pumping rods with the well set up to be produced by the means of a pumping unit, there are no production lines hooked up and the pumping unit and well head were found completely covered in vegetation. A call was placed to Dave Secrest the owner of D-D Oil to confirm the well is correctly identified and plotted correctly. The lease owner and operator Dean Christman indicated this is the Hawkins lease.
