1103828407 2016-10-17 Production Wells No Violations Status check inspection found the well equipped with a 3.5 casing liner string that is reportedly stuck in the hole. There is a 15.5 long piece of 3.5 tubing with a 2.875 short piece of tubing welded to it laying on the ground near the well. It appears as though the tubing was attempted to be pulled up out of the well bore and was cut off at ground level and a new 3.5 collar welded to the tubing still in the well. There is a 1 plastic flow line hooked to the well head.
1480514851 2016-11-30 Plug / Plug Back No Violations On this day I met on site with Troy Harmon who has been contracted to plug the said well. Upon inspection it was observed that the well was equipped with 18 wooden conductor along with a 5.187 casing string and a 3.5 tubing string set at some point as a liner. Other information provided from previous contractors who have worked on the well indicated the 3.5 was set on a packer and the entire 3.5 consist was hung in the hole and unable to be pulled, at some point an attempt was made unsuccessfully to shoot the 3.5 off above the packer. Directions were given to Troy Harmon to log the 3.5 to determine if there is a shot hole present in the bottom of the well and if total depth can be achieved, while logging the free point of the 3.5 needs to be determined to determine a plugging plan. Due to the ground level being graded next to the well head during the building of the Gulfport Claugus well pad a cellar will be set around the well head and sealed at the bottom to control fluid flow from the well. A plugging permit has been applied for by the well owner.
1481654057 2016-12-13 Production Wells No Violations Status check inspection to evaluate the 3.5 diameter tubing string set in the well bore. A service rig owned by Harmons Well Service is rigged up on the hole and JET Wireline has run a Collar Locator-Bond Log with the wireline tools unable to get to bottom setting down at a measured depth of 1,279. The Bond Log showed product in between the 3.5 and 6.250 casing strings. The service rig will attempt to run a 1 tubing string to the well bottom. JET wireline indicated that several years ago he had previously attempted to shoot the 3.5 unsuccessfully and the 3.5tubing was hung in well and was unable to be pulled with a service rig.
1481827450 2016-12-15 Plug / Plug Back No Violations On site inspection to witness the well plugging process with a service rig on the hole owned by Harmons Well Service and Formation Cementing. The well is equipped with 3.5 diameter tubing and 6.250 diameter casing with no records of depth on either string and the 3.5 is stuck inside the 6.625. JET wireline did run a Gamma Ray Collar locator log on the 3.5 on 12/13/2016 with the logging tools not being able to get past 1,278 and the 3.5 showing bond to surface. The service rig was then successful in being able to run a 1 tubing string in the well to a total depth of 1,531. The 3.5 was loaded with fluid prior to logging the well by Harmons well svc with 80 barrels of fluid indicating a possible shot hole completion. Formation Cementing pumped a 100 sack 18 barrel total mix bottom plug through the 1 tubing with 10 barrels of 6% gel spacer ran ahead of the cement. The 1 tubing was pulled up to 1,100 and the hole circulated clean with cement and gel circulated to surface. JET wireline service along with Formation Cementing will be on location the next morning to attempt to perforate the 3.5 diameter tubing and 6.250 casing and circulate cement into the annular well bore if present.
1532691443 2016-12-16 Plug / Plug Back No Violations On site inspection to witness the well plugging process. JET wireline service is on location and ran in the hole with wireline and tagged the previous plug set at 1,448. The service rig then ran 1 diameter tubing back in the hole to 1,443 and set a 35 sack plug. The tubing was pulled and JET wireline ran back in the hole and shot 6 perforations across the coal seam at 490 to 495 and a collar buster at 300 to attempt to free the 3.5 tubing. The service rig then ran 1 back in to 1000 and a plug was pumped to isolate the middle section of the well bore. The 1 tubing was then once again pulled and the 3.5 attempted to be pulled from the well bore unsuccessfully. At that point the 1 was then run back in the well and a surface plug pumped from 300 to ground level with the well circulating cement inside and outside the casing strings to surface indicating internal and external cement placement. The cement top was dropping back to approximately 40 from surface and the company will resume activities on the next working day and top the well bore off accordingly.
1667957686 2016-12-16 Plug / Plug Back No Violations During my inspection Formations Cementing was on location along with Jet Wireline Service. I witnessed Formations Cementing pump 37 sacks of cement with 1 percent calcium through 1 inch diameter tubing to top off bottom plug. I was accompanied by ODNRs Inspector William Stolarik.
