1524164331 2018-04-19 Production Wells No Violations Status check inspection found the well equipped with a wooden conductor string along with a 6.625 diameter production casing string, the 6.625 diameter string well head has been cut off and a 4.5 diameter casing has been set in the well bore as a liner. There is a 2.375 diameter production tubing string set inside the 4.5 casing liner with pumping rods inserted into the 2.375 production tubing string. There is a casing and tubing well head present with production flow lines attached to both well head sections, only the casing head has a production valve present. On the property in the area of the well head a single 100 barrel steel construction tank is present. The tank has a gravity fluid separator sitting atop the tank with 2 discharge lines present, one to allow the oil to go into the tank the other to allow the production water to be discharged onto the earths surface a short distance from the tank. Below the discharge point is a area where no vegetation is present and the trees have died. Photographs taken on this day.
1524512623 2018-04-23 Production Wells No Violations On this day I met on site with the well owner Dean Christman along with ODNR District Supervisor Brent Bear and Inspector Tom Thomas, also on attendance was ODNR Hydrologist Scott Scheiner and Emergency Response personnel member Nathan Lane. The inspection was a follow up to an issue I observed on 04/19/2018 regarding the discharge of well bore production fluid onto the earths surface. Upon arrival it was observed that Mr. Christman has began remediation of the affected area over the weekend. A remediation plan of action was discussed and agreed upon by all parties on this day. Mr. Christman was bringing in a second piece of equipment and would continue to restore the affected area. The production fluid area has been repaired with an attempt to make a liquid tight dike and a second fluid tank has been set to recover any production fluid created. Mr. Christman was given a timeline of 5 days to remediate and restore the site. Also discussed was the depth of the said well and a Gamma Ray Neutron- Casing collar -perforation log was provided to myself by Mr. Christman. The log was from 10/31/1979 and indicated the 4.5 production casing was set to a total depth of 2,230 from ground level elevation. The log indicated the 4.5 production casing was perforated between he depths of 1,389 to 2,218 in seven different intervals. No completion record or log is on hand with the Division and I shall submit the log and return it to Mr. Christman.
1525109740 2018-04-30 Production Wells No Violations Inspection follow up to confirm the produced has remediated the area below the production facility previously affected by the brine discharge. The area below the production facility has been remediated and seed has been applied to the entire area. The landowner who also owns the well identified as Dean Christman also has applied manure to the area to help establish vegetation.
1529061497 2018-06-15 Production Wells No Violations Status check inspection found the producer has remediated the area below the tank battery where production fluid was previously discharged and has established vegetation in the affected area. An liquid tight earthen dike has been established around the production tanks. The well is equipped with multiple casing string and the producer confirmed it was deepened at one point as the previous permit indicated. No records were initially submitted but I did receive a wireline log and submit it to permanent records for the well.
1890012493 2018-04-23 Production Wells No Violations On this day, I accompanied ODNR Inspector William Stolarik, Southeast Regional Supervisor Brent Bear, ODNR Hydrologist Scott Scheiner, ODNR Emergency Response Nathan Smith, and well owner Dean Christman. Please refer to Inspector Stolariks comments for a detailed report. Kerry Brown was also informed of the situation and explained the procedure to remedy the issue.
