I informed Chuck Dye who is working as a representative for B.F.Paulus that no temporary owner number had been applied for and no plugging permit had been applied for. I informed Chuck I needed to see some action on this issue to resolve the complaint # 3077 filed against the producer B.F. Paulus by the landowner Jeff Allen.

1043997957 2015-04-14 Production Wells No Violations Follow up inspection to complaint # 3077 and violation # 130500191. Upon inspection of the well it was observed that the 6.625 casing was found standing open with 2 production tubing present. A gas relief packer style well head is present. There is no pumping rods in the well bore and no flowlines are hooked up. The production facility has been removed. Photographs were taken on this day.

1181979469 2017-02-06 Production Wells No Violations Administrative Report to document that I have had multiple conversations with B.F Paulis who has indicated he was producing the unregistered well and has through the due process of the law lost his right to produce the well and minerals on the lease. It was also of my understanding that when the landowner Jeff Allen and B.F Paulis was in court the ultimate outcome of the well was not addressed. I have also spoken with a local contractor identified as a Chuck Dye who indicated he has been paid by B.F Paulis to complete the well registration and plug the well. The landowner Jeff Allen has also called multiple time for an update on the well status. I have informed both the landowner Jeff Allen and the acclaimed producer B.F Paulis that in order to move foreword and plug the well the well would need to be registered and a plugging permit issued, in which neither has been applied for.
1276485502 2014-01-23 Production Wells Violations Noted Status check inspection found the well idle and incapable of production along with no current owner registered with the Division and no reported production in the database. The following information has been provided to the Division, An Affidavit of Forfeiture Under O.R.C 5301.332 found in the official records of the Monroe County Ohio Recorders office under Volume 0209 page 657 - 658 & 659 along with a signed Assignment of oil & gas lease signed by B. F. Paulis showing the transfer of the lease to Tyrone J. Allen and Diane L. Allen of 62982 Arrow head Road-Cambridge Oh 43725, Instructions are as follows Obtain proper Owner Bonding and correctly register the said well with the Division within 60 days, make the well capable of production within 120 days or file for a plugging permit with the Division. 1279356386 2014-04-28 Administrative Inspection No Violations On the date of 4-28-14 Jake Glasscock sent email stating that the well is api# 111-62098. The Original owner was Crawford Bros.and Company.
1345414270 2014-02-18 Administrative Inspection No Violations On this day I was contacted by phone by B.F Paulus in regards to the violation issued against the said well, I informed Mr. Paulus that He by admission was the last known producer and He was responsible to plug the said well. I also informed Mr. Paulus that I also have reviewed the legal documentation showing statements by Paulus claiming to own the lease, I also spoke with my supervisor Rocky King and Erica Freeman in the permitting section in regards to the matter, Mr. Paulus has contacted the permitting section in regards to the matter.
1390315120 2011-04-04 Production Wells No Violations I met with complainant Jeff Allen on old well. The complaint is based on complaint # 3077. There is an old well with 6.625well head and a gas escape packer. There is a pumping unit complete with electric hookup. It appears that there is no tank or flow line. The 2or the 1tubing are not connected. There is an unknown owner. Jeff Allen will have Attorney look for owner in the courthouse research. I will call Central Power.
1390322631 2014-01-16 Administrative Inspection No Violations I called Jeff Allen on well. He said that Attorney did take over the lease and minerals. The well is still owned by B.F. Paulus. He would like to get well plugged. He will send all pertinent information to me by e-mail.
1390395162 2014-01-21 Administrative Inspection No Violations I talked to current inspector [Bill Stolarik] on this well. I will meet with him and do a current inspection. Then he will issue a compliance notice. The landowner Jeff Allen will send info by E-mail.
1390500191 2014-01-23 Production Wells Violations Noted Follow up inspection to complaint # 3077. On this day a on site inspection was done with inspector Pat Shreve and the following equipment was found at the well head-7 casing- 2 tubing with a gas escape 1 & 2 tubing string well head with only the 2 tubing inserted in the well bore with a valve in the closed position on the tubing and no 1 tubing present, The pumping rods and 1 tubing are present laying on the ground besides the pumping unit. No annular casing valves are present and no production lines are hooked to the well head. The well is incapable of commercial production in its current state, No production tank or gas metering equipment was found. The well owner is not identified at the well head or in the database and no production is reported in the database. Instructions are as follows File for a temporary owner number and a plugging permit with the division within 30 days, plug the said well within 120 days. ODNR contact Inspector Bill Stolarik 740-507-6592.
1390505797 2014-01-23 Production Wells No Violations Bill Stolarik and I inspected well and revealed the pumping unit is unhooked from the well. The 1tubing and rods are pulled from the 2 and laying on ground. There is a 7casing and a old gas escape set up at well head with 2 tubing still in the wellhead. The electric is on pole but incapable. Well is not hooked up for commercial or domestic production. The well is currently idle and unregistered. There have been no production file with the State. B.F.Paulus will be issued a Compliance Notice by Bill Stolarik.

Mr. Allen explained the Affidavit of Forfeiture proceedings and the fact that although He now is the lease holder He believes that B.F Paulus is still the responsible party of the idle well on His property, Mr. Allen requested a meeting and I informed Him that I did not feel comfortable discussing legal matters and that we would continue to address the last known party attempting to produce the said well as the responsible party, In this case that would be B.F Paulus, Mr. Allen said He would mail me the rest of the documentation He had on file including the original lease and tax statements, I once again advised Him that a legal opinion in this matter would have to come from a different party within the Division.
1392896243 2014-02-20 Administrative Inspection No Violations Due to recent development updates and unusual circumstances regarding the unregistered producer known as B.F Paulus the original violation has been updated from 062.A to 12.B, The documentation provided by the landowner shows B.F Paulus has lost His legal right to produce the lease due to a legal matters. Paulus has been advised He is responsible to plug the said well. I have in good faith updated the original violation dates giving Paulus ample time to file for a temporary owner number and plugging permit with the Division and complete the plugging process.

I on this day copied all inspection and complaint records and sent them to my District Supervisor Brent Bear so he may follow up with the Division permitting section to see the status of the unregistered owner B.F Paulus who the violation was issued against. A current active complaint is logged in the database under complaint 3077.
1535388247 2018-08-27 Production Wells No Violations Follow up inspection to complaint # 3077 found the well equipped with a 6.625 production casing string with a bowl style well head attached to the top of the casing string. The well head bowl is equipped with a dual string insert however only one 2.375 diameter non upset tubing string is present at surface. There are no casing valves or flow line on the bowl of 2 diameter tubing string. The 2nd side of the bowl insert is standing open also. There is a pumping unit standing over the well however it is not connected. The power line running to the well is disconnected and the main service line running to the well has been severed near the well. No production flowlines lead away from the well and no production equipment is present on the property identified as associated to the well. Photographs taken on this day.
