On this day I spoke with Bobby Harmon who is a contract producer for CGO Inc, Ms. Harmon indicated she would address the identification issue.

0698670867 2015-07-20 Production Wells No Violations Compliance Notice # 1372983655 follow up inspection found the well has not been registered with the division per my previous requests, I have provided the said producer Ed Crum with all proper identification to register the said well known a 34-111-6-3157-0000, Neibsch John #3. Upon inspection of the well it was observed that a 1 tubing string has been placed inside the 2tubing string with a 1 sales line attached to the said well, the producer has installed a check valve. No gas sales meter is present to demonstrate the wells production, there is a combined sales meter present for all the wells in the area owned by the producer,at the commercial sales point where the producer sells gas into a Gatherco owned Roots 8-C rotary index meter that is currently showning a meter reading of 544 on the rotary index chart.
0800225123 2013-07-29 Production Wells No Violations I received notification from Bobbi Harmon that they have moved on to the said well with excavation to prep the area to bring in their service rig to evaluate the said well. I responded and witnessed Harmon well service excavating the area around the well, In discussion with Troy Harmon we went over the down hole evaluation work to be done, troy stated that they were going to submit a plugging application and they would decide when the rig was on the hole if they would plug or produce.

1146438820 2014-02-24 Production Wells Violations Noted Status check inspection found the well has not been registered with the division per my previous requests, I have provided the said producer Ed Crum with all proper identification to register the said well known a 34-111-6-3157-0000, Neibsch John #3, It was also observed that a 1 tubing and flow line has been installed with no inline check valve to prevent back feeding gas into the well bore thus avoiding Annular Over pressurization, and no means of monitoring the gas sales has been installed per my prior request to demonstrate the well is of future futility . The said well also has a unresolved violation number 1352823287 issued on 11/13/2012.
1352294621 2012-11-07 Administrative Inspection No Violations Inspection and status check follow up on a unregistered well, During August of this year Ed Crum had Harmon well service attempt to repair the said well. The rig has since moved off the hole and the well is idle. The well is equipped with 6.250 casing and 2 tubing. Inspector made contact with Ed Crum on 11/06/2012 and requested that He register the said well and cease any mechanical activity on any unregistered wells.
1352823287 2012-11-07 Production Wells Violations Noted Idle well that shall be plugged due to being incapable of production due to the absence of production equipment and the sales line not hooked up. Inspection of the said well found well idle and no identification present with the well not registered on the producers bond and no reported production. Inspector did witness well owner with a service rig on hole in August of this year. Well sits in a streambed.
1355758130 2012-12-17 Production Wells No Violations Recent phone conversations with the well owner Ed Crum found that Mr. Crum is still persistent about putting the well into production. Inspector will note that the Violation wrote on 11/13/2012 was a plugging order and the Inspector has advised Mr. Crum several times that the well is located in a waterway and is not to be disturbed until the EPA and the US Core of Engineers and any other Government Agency that may have jurisdiction is properly notified. Inspector did visit well site on this day.
1373983655 2013-07-16 Production Wells No Violations Follow up inspection to plugging order wrote on 11/13/2012 against the said well owned by Ed Crum of CGO Inc - producer # 2849. On this day I am giving the producer 30 days to file with the division the proper plugging paperwork application or I shall proceed with a request for a Chiefs order to suspend the operators Bond and production privileges.
1383759567 2013-11-06 Production Wells No Violations Compliance agreement follow up found Harmons well service rig on the hole but not working. I met with Ed Crum in the area and He indicated they were going to attempt to set 4.5 casing on a gas escape packer below the 6.625 casing in the open hole original well bore section and seal off the migrating water from above.
1386945317 2013-12-12 Production Wells No Violations Compliance agreement # 1372983655 found the service rig has moved off the hole since my last inspection and the work over pit has been left open, A 1 tubing string has been inserted inside the well bore and in conversation with Ed Crum on Wednesday December 4th He commented that they had repaired the well and it looked like it was going to a good well, I however found the pressure on the tubing to be zero on this day, Inspector Pat Shreve was with me on this day & I will also will ask for opinion from my supervisor on this issue, A plugging order was filed against the well due to it being idle for many years and the wells geographical location in a stream, The producer Ed Crum has hired a excavation contractor and altered the stream bank removing the well from the stream area.
1389111040 2014-01-07 Administrative Inspection No Violations Administrative inspection to document the follow up of the unresolved violation and CGO not properly registering the said well. On this day I called Bobby Harmon who takes care of the paperwork for Ed Crum and advised Her they still needed to register 2 wells - 34-111-6-3157-0000 & 34-111-2-1043-0000, Bobby said She would complete the registration form 7 and submit it to Ed Crum so He could write the check and sign the paperwork to be submitted to the division.
1567368405 2015-08-13 Production Wells No Violations Compliance Notice # 1372983655 follow up inspection found the well has not been registered with the division per my previous requests. I have provided the said producer Ed Crum with all proper identification to register the said well known a 34-111-6-3157-0000, Neibsch John #3 . With the following equipment being observed at the well head on this day, A 2 tubing string is present serving as the producing string with a 1 valve present on the sales line in the open position feeding into a 1 gas sales line. No well identification is noted as present. No sub gas meter is present on the said lease.
1672511879 2014-07-10 Production Wells No Violations Compliance follow up inspection of the well head found 1 tubing has been installed in the 2 tubing with a 1 flowline hooked up and the production valves in the open position, database records show the well has not been registered in the producer known as CGO Inc Ed Crums name and no production has been reported.
1903112137 2012-11-28 Production Wells No Violations Inspector met with Ed Crum on location and discussed the violation issued against the said well. Mr. Crum stated He would like to start the rework procedures and I noted to Mr. Crum that per the conversation I had with Mike McCormac, who is the division supervisor over permitting that He must first register the said well. Inspector will note after comparing the identification of the well to the well Mr. Crum has on His bond that it is quite possible the well is registered under a improper permit number which is 3411163153. Conversation also took place concerning the close proximity of the well to the stream and the rework procedures Mr. Crum has in mind.
