On 08/01/2016 violation # -1877750021 was issued due to the oil production tanks being allowed to overflow into the containment dike area. Upon inspection it was observed that oil could be visually seen standing on the surface area inside the containment dike area and the ground inside the dike was saturated with oil. One of production tanks inside the dike was observed to be full to capacity with oil along with the 1600 gallon plastic water tank. In phone conversation with the well owner Andrew Allphin on 08/02/2016 I was informed he has contracted a crew to clean up the affected area and on this day a Robert Hesson with H&L Water service contacted me and indicated he would be on site at 8:00 am on 08/03/2016 to recover the product product with a vacuum truck and remove any affected soil to a EPA approved landfill. |
1877750021 | 2016-08-01 | Production Wells | Violations Noted | On site status check inspection found the well equipped with 6.625 casing- 2 production tubing with the well set up to be produced by the means of a pumping unit with pumping rods installed in the well and the production valves in the open position. At the production fluid storage area 2-70 barrel production tanks along with a 1,600 gallon plastic tank is present inside an established dike area. Also present inside the dike on the earths surface is crude oil with the soil inside of the dike being saturated, there is also a affected area 3 in width x 20 in length below the dike where the oil product has escaped out of the dike. A fluid gravity separator is also present above the dike area. Instructions are as follows - recover the oil product from within the dike and remediate the affected areas accordingly. On this day I witnessed H&L Services remove the product from the production tanks and remove the production tanks from the dike area. The contractors will be on site the next day to solidify the area inside the dike that is affected by the oil spill and remove the affected material. |

Lime was spread across the affected area inside the dike and H&L Oil Field Service brought in a single axle dump truck load of sawdust and applied it to the affected area inside the dike. The affected soil was then solidified with the sawdust and lime. The soil affected was then loaded into the dump truck and hauled from the area and placed on a plastic pit liner for temporary storage until a roll off box arrived, the material is then to be hauled to a EPA approved landfill. The production tanks were then reset into the containment dike. The company will then reseed and mulch the disturbed area. Photographs taken on this day and no further clean up action required. |
