0203282456 2016-07-26 Plug / Plug Back No Violations My inspection today finds Rob Armstrong is providing the plugging services. Mr. Armstrong pulled 1494.6 of 2 tubing out of the well. Carper Well Service is on location with a load of water and a load of prepared clay.
0601218558 2016-08-04 Plug / Plug Back No Violations On site inspection to witness the well plugging process with prepared clay product. The plugging contractor is Rob Armstrong and the plugging commenced on 08/01/2016 with the last working day being 08/02/2016 at which time the top of the clay plug was tagged at 1,012 when work was stopped for the day. On this day the clay top was tagged at 982and plugging was continued with the top of the clay plug being tagged after every bucket of clay is applied to the well. The clay is being loaded into a 3 cu/yd. farm tractor bucket and hand mixed into a trough. The well bore continues to settle as more product is applied with a total of approximately 8 tons applied by the end of the working day and the well bore top of clay tagged at 1,036 indicating the shot hole is filling up.
1321974232 2016-08-08 Plug / Plug Back No Violations My inspection today finds the plugging operator Rob Armstrong tagged the top of the plug at 52 from surface. Mr. Armstrong is going to wait for the plug to settle one additional day before pulling the 5 liner string and finishing the plugging operations.
1427675801 2016-07-27 Plug / Plug Back No Violations On site inspection per request of the Plugging contractor Rob Armstrong who has been rigged up on the well for 14 days. Upon inspection it was observed that the 2 tubing and pumping rods have been removed from the well and a tally taken of both indicating a total depth of 1,500. It was also observed that there is a 5.187 casing string liner of unknown length set inside the 6.625 production casing. Mr. Armstrong indicated over the phone on the evening of 07/26/2016 that He would commence plugging the next day, however upon arrival Mr. Armstrong indicated he did not have His well bore bailer or equipment to mix the clay. Mr. Armstrong said He would return the next working day and commence bailing the well before begin to plug with clay. Mr. Armstrong was advised to bail the well from the bottom up to approximately 1,000 before attempting to recover the 5.187 liner.
1452782089 2016-01-14 Production Wells Violations Noted Status check inspection found the following present at the well head - A undetermined casing string not visible at surface along with a 2 production tubing string and pumping rods with the well set up to be produced by the means of a pumping unit, attached to the 2 production tubing head is a 1 flow line with no valve present. Attached to the casing head is a 2 flow line reduced down to a 1 gas sales line with the production valve in the closed position. A single 100 barrel production tank was present inside an established dike. A electrical meter near the well head indicated no recorded sales and the condition of the well indicated no recent production activity supported by the lack of reported commercial production to the Division since the year 2010. No well identification was present. Photographs were taken on this day. Instructions are as follows- The well owner/operator shall submit a written plugging schedule to (Bill Stolarik) at (5880 Memory Road Zanesville Ohio 43701) within 30 days, per OAC 1501:9-11-05(B), and plug the well within 180 days. The well owner/operator will be notified if the plugging schedule is approved or denied.
1462040939 2016-04-30 Production Wells No Violations Received phone call from county inspector Bill Stolarik stating that there was a reported oil spill in Monroe county that needed someone there to witness cleanup of contaminated soils,cleanup was done by Rob Armstrong and Carper Well Services provided roll off box to haul away soil to a approved landfill told them to submit disposal ticket to county inspector upon request.

Note:Well is equipped with pump jack and 1-100 bbl. Tank which has hole in bottom of it.

1462812706 2016-05-09 Production Wells No Violations Compliance Notice #1452782089 follow up inspection found the producer has applied for and received a plugging permit with the Division and submitted in writing a plugging scheduled. The 100 barrel oil tank associated with the said well did leak the product out of the tank on 04/30/2016 and upon inspection it was observed that the majority of the oil had been recovered out of the dike area with only a slight amount of residue left inside the dike. I stopped at the residence of the well owner Larry Christman and he will use oil absorbent rags and recover the residue and place the remainder of the rags under the 100 barrel tank to absorb and remaining residue that may leak out.
1468524244 2016-07-14 Production Wells No Violations Compliance notice # 1452782089 follow up inspection found the pumping unit has been removed from the well- the location graded and a service rig has been mobilized and is standing rigged up on the hole. A plugging permit has been issued and the well is slated to be plugged.
1470073913 2016-08-01 Plug / Plug Back No Violations Upon inspection on this day it was observed that a local contractor identified as Rob Armstong is on location with his service rig and is bailing the well bore fluid down and has the fluid level witin 40 of total depth of 1,500. The well is believed to be a shot hole completion and plugging with approved clay will commence tomorrow,08/02/2016.
1470188831 2016-08-02 Plug / Plug Back No Violations On site status check inspection accompanied by ODNR Supervisor Brent Bear at 13:00 pm. Upon inspection it was observed that a service rig was on location owned by Rob Armstrong - Mr. Armstrong was on site and mixing a clay-water mix in a trough and gravity feeding the clay mix down the well bore. The fluid level in the well bore had been previously lowered by the use of a dump bailer. The clay mix was being housed in a roll off box and carried to the mixing trough in a 3 cu/yds. farm tractor bucket with Mr. Armstrong indicating he was on his 3rd bucket of clay and the well appeared to have a shot hole in it at approximately 1,000. At 15:00 pm we left the site and Mr. Armstrong was going to apply another 1/2 bucket to the well and let it settle for approximately 2 days to ensure the material has adequate time to settle out in the shot hole. The previous violation shall be considered resolved upon the completion of the plugging project.
1470448474 2016-08-05 Plug / Plug Back No Violations On site inspection to witness the clay plugging ongoing process. Upon inspection it was reported by the plugging contractor Rob Armstrong that the top of the clay inside the 9.625 was left at a measured depth of 218 at the end of the previous working day and tagged at 982 on this day. The original shot hole appears to be at 1,000 to 1,100. A total of 10 tons of prepared clay has been administered to the well with the clay now beginning to fill up into the 6.625 casing string. An additional 10 tons of clay was brought in on this day and the well was topped off to ground level before the rig crew quit for the day. The top of the clay will be checked the next day and if the level stays to surface the rig will move over and begin to plug a 2nd well on the property for the owner. We will watch the well bore for several weeks and top off as needed before permanently closing the casing top in.
1470620803 2016-08-07 Plug / Plug Back No Violations Status check inspection on the well plugging process found the service contractor Rob Armstrong on location applying prepared clay to the well bore with approximately 16 tons of clay used to date. Also noted is that the last 4 working days have found the clay level inside the well bore falling back overnight to between 971 to 978 from ground level the previous day. The contractor is keeping the fluid level bailed down to the top od the clay level. The well bore is showing the indication of possibly having 2 shot holes.
1470759734 2016-08-09 Plug / Plug Back No Violations On this day I witnessed the service contractor Rob Armstrong tag the top of the clay level in the hole at 32. The clay level will be brought up to 20 inside the well bore today and the well will be left open for 7 to 14 days to monitor that the top of the clay does not fall back down the well bore. At that time a 20 cement plug will be placed from 20 to 30 and the 6.625 casing cut off and a steel plate place as a permanent marker. Inspector Eric Conrad and myself have monitored the entire plugging process with on site confirmation daily with a total of 18 tons of prepared clay administered to the well bore total.
1473557927 2016-08-06 Plug / Plug Back No Violations I am onsite to witness the plugging operator, Rob Armstrong, tag the clay plug that was installed the day before. Mr. Armstrong ran a bailor to a depth of 971. The rig crew intended to bail off all free water from well before attempting to wash in more clay.
1943322452 2017-01-23 Final Restoration No Violations The well site meets ORC 1509.072. No pit was dug during the plugging process and the area has been seeded and mulched with vegetation established. The well head was cut off and a maker installed 30 below the ground.
