0199132959 2018-04-19 Production Wells No Violations Compliance notice # 1485795215 follow up inspection found the well equipped with casing tubing and rods and set up to be produced by the means of a pumping unit. A new production facility has been set consisting of a single 100 barrel steel construction production tank along with a plastic 1,200 gallon tank used for production water. The landowner previously identified as the individual producing the well for domestic purposes has yet to register the said well into his name.
0753896982 2017-10-04 Production Wells No Violations Follow-up to Compliance Notice #1485795215. On the day of my inspection I found (1) 100 barrel steel tank and (1) 1,550 gallon polymer tank all contained within an earthen dike , and shared production from 3411165801. Dead vegetation around the base of the polymer tank indicated possible release of brine within the immediate area. Upon further investigation, the 2 inch diameter valve at the base of the tank was in the open position with a cap over the end of the opening which was continuously dripping at a rate of one drip per second. Landowner John Burkhart approached me in the field and he informed me that he would address the issue immediately. The wellhead is equipped with 6 5/8 inch diameter casing and 2 inch diameter production tubing with a pumping unit and Predator Engine.
1109089276 2017-02-06 Production Wells No Violations Status check inspection found the well equipped with 6.625 casing and 2 production tubing with casing and tubing well heads present and production lines hooked to both the casing and tubing with production valves in the open position. The production facility is being relocated on the day of my inspection and is not complete. The well is being produced for domestic purposes by the landowner John Burkhart. The well API number has recently been assigned with no record of a well present at this location historically. The landowner has been sent a Domestic registration packet by the Divisions Surety and Bonding section. The well was reportedly drilled CIRCA 1898 by the Pure Oil Company and is producing from the Lime section at a approximate depth of 1,000 and was part of the original McVeigh lease.
1176691067 2017-04-09 Production Wells No Violations Compliance notice # 1485795215 follow up inspection found the product from the oil spill that occurred on 01/29/2017 has been recovered and the tank battery reconstructed inside an earthen dike. Identification is posted on the production tank. The Division has submitted registration packets to the well owner John Burkhart.
1290186799 2017-02-02 Production Wells No Violations Status check inspection of the recent oil spill that occurred on 01/29/2017. On this day I traveled along the Little Muskingum River From County Road 15 to the well site on Ohio State Route 260 above Marr Ohio. There was slight signs of Sheen present in various locations above the Hard Boom recovery points. The unregistered well owner John Burkhart has recovered any standing fluid from on the property and began reconstructing the production facility. Mr. Burkhart has been advised that He must be in compliant with the laws regarding the production, storage and disposal process of being a well owner along with completing the well owner registration process.

1485874906 2017-01-31 Production Wells No Violations On this day I arrived on site to witness the fluid recovery process of product inside a underground 8 clay tile and lying in a defined ditch line, with Harmons Well Service crews on location utilizing a vacuum truck and manpower. At 8:36 am I arrived on location along with the Crews from Harmons Well Service, we positioned their vacuum truck on the property of John Burkhart in a area near the Clear Fork of the little Muskingum River identified geographically as a coordinates of EPE 15-39.64362 X 81.23576. At 9:03 am the product lying in the defined ditch line was removed the end of the clay tile uncovered and the product was removed with crews drawing up the oil with the vacuum truck as it migrated out of the end of the clay tile. At 11:00 am a fresh water flush was began by introducing 750 gallons of fresh water into the clay tile next to the production tank at a geographical location coordinates of EPE 13-39.64449 X 81.23524. At 12:20 pm a second water flush of the clay tile was initiated. The earthen product that had been in contact with the product in the defined ditch line was shoveled up and placed in garbage bags. The crews continued to evacuate the clay tile through out the day removing approximately 70 barrels of mixed product from the tile. There was also a area around the tank battery that was dug up and the soil removed and to hauled to an EPA approved landfill. The tank battery will be reconstructed with a earthen dike and additional production tank. Crews from Harmons Well Service are also walking the entire length of the affected area of the Clear Fork Little Muskingum River with a small boat in tow, the men are mopping up any standing oil found in the waters of the State.
1487182913 2017-02-15 Production Wells No Violations Status check inspection found the well owner had constructed a production facility consisting of a steel 100 barrel production tank and a plastic 500 gallon tank. The owner has also constructed an earthen berm perimeter around the production facility.
