1012943197 2019-03-04 Plug / Plug Back No Violations I was on location today to witness the plugging of this well. The rig crew Keith Metheny was on site to try to retrieve the tools that was lost down hole. The dozer that was on location got pulled from this job so we had to wait on a dozer to come out so we could get the tubing on location. I was told the dozer would be here by 9:30 am but they had problems. The dozer did not get on location until later in the afternoon. We ended up shutting the job down until the next day.
1081425528 2019-01-09 Production Wells No Violations I was back on location today to follow-up on this well. I spoke with Doug today during my inspection. He is still planning on having this well plugged by the end of February.
1094742793 2019-02-28 Plug / Plug Back No Violations I was on location today to witness plugging operations on this well. Keith Matheny has put his service rig on the well. Mr. Matheny attempted to get to bottom yesterday but could only get to approximately 480 feet with 2 inch tools. During my inspection Mr. Matheny ran 4 inch stem and jars ran into an obstruction at approximately the same depth. As he began to pull the tools out of the well the cable snapped and lost his 4 inch tools in the well. Mr. Matheny said he would be back tomorrow with tubing and fishing tools. 1133969433 2017-06-02 Production Wells No Violations I am accompanied by Dan Goins. The company has made no attempt to comply with the Compliance Notice. The well is incapable of production. Production has not been reported since 2004.
1466107206 2016-06-15 Production Wells No Violations I inspected this well today and found it with no wellhead and open to the atmosphere. Fluid level was estimated to be @ 450. The well consists of 4.5 diameter production casing with no additional casing/tubing visible. There were no tanks or other production/sales equipment found associated with this well. I spoke with Randy Adkins, owner of the property (740-516-0539), and he has no record or information regarding who the wells belong to. He was told by Mr. Pinnell who sold him the property in 2004 that the wells had been released. Adkins also stated that the man who was recently out on the property to work on the wells (he did not remember his name) told him that they had swapped some leases with Kilbarger and that was how these wells had become theirs. A previous inspection conducted by DOGRM Inspector Pat Shreve on 10/8/2015 stated that Doug Kitchen had been out on the property to plug the wells and had removed equipment from 2-3 wells. A search at the courthouse found no ratification of lease signed by Randy Adkins who bought the property in 2004. I searched courthouse records under B & B Enterprises, Doug Kitchen, Paul Grim, Hocking Hills Energy, Chieftain Energy, Ray Branham (all known to be associated with Mr. Kitchen) and found no Bill of Sale, lease or assignments to any of these names. Based on the available information, there does not appear to be a legal owner of the wells and equipment.
1506431333 2017-09-26 Production Wells No Violations I was on location today to do a follow-up inspection on this well. At this time the well is still idle and incapable of production. The well sits here with 4.5 inch diameter casing at the surface about 3 feet high. The well is open to the atmosphere with no way of containing the well if something would happen. The vegetation is grown up around the well making it hard to find. The well is about 15 feet from the lease road. I have taken pictures of the well today for my inspection report. The lease road is open to the well for access.

1695970138 2019-03-05 Plug / Plug Back No Violations I was back on location today to witness the plugging of this well. Today the rig crew was able to run the 2 inch diameter tubing in the well with a fishing tool on it to retrieve the 4 inch diameter tools and cable that was lost when checking bottom. The well appears to be bridged off at about 500 feet and is filled with clay. I think the 4.5 inch casing is parted down hole. There is no other casing in this well so the 4.5 inch casing will have to stay in the well. The rig crew is running the 2 inch diameter tubing back in the well to see if we can clean the well up and get the tubing down to bottom to plug the well. After discussing what we would do with this well we decided to setup Appalachian up to come out to log this well. They are setup for tomorrow morning.
1985728993 2015-10-08 Production Wells Violations Noted Today on my inspection I was accompanied by Inspector Shreve. We found this well has no pump jack or lines connected to it. The well was issued a permit to drill the well on07/15/82 and from then until the present time the well was drilled. The well is only a 4 1/2 casing sticking out of the ground with some fluid in the casing and the well has no identification. It is not capable of production. Well identification must have: WELL NAME, PEMIT NUMBER, COMPANY NAME, COUNTY, EMERGENCY CONTACT INFORMATION. The well owner/operator shall submit a written plugging schedule to Inspector Sefcik at 5880 Memory Road Zanesville Ohio, 43701 within 30 days, per OAC 1501:9-11-05(B), and plug the well within 180 days. The well owner/operator will be notified if the plugging schedule is approved or denied.
2062212397 2019-03-06 Plug / Plug Back No Violations I was back on location today to witness the plugging of this well. We had Appalachian come out today to run a bond log down the well to make sure there were no voids behind the 4.5 inch diameter casing. After running the log we found that there is only 450 feet of 4.5 inch casing in the well. We ran the tubing back down to 450 feet and cemented the well to the surface using Class A cement from Formation Cementing. A total of 60 sacks of cement was used to fill the 4.5 inch casing to the surface.

I will contact Bill Mitton to see when he will be able to plug it. I talked with Bill and he said he is just waiting to get the okay on plugging it. I then talked with Doug Kitchen and he stated he will have Knox Energy on the well by late November early December. He said the well will be plugged this year.
