0322244887 2015-11-06 Plug / Plug Back No Violations On location to witness plugging operations for this well. Upon arrival, Nicks Well Plugging (NWP) was preparing to rip 4.5 casing. Bond logs show top of cement on the 4.5-7.875 annulus at ~3500, which is above the top of the Big Lime. The 4.5 casing was ripped at 2936 and 1 joint was pulled. NWP set a rip isolation plug from 2894-2694 using 60 sks class A cement and 43 bbls of freshwater displacement. NWP then pulled 27 joints of casing to 1780 and set a Berea Sandstone/bottom of surface casing plug from 1780-1310 using 137 sks class A cement, 100 lbs LCM, and 18 bbls freshwater displacement. NWP then pulled casing to 200 and circulated a surface plug using 65 sks class A cement. The remaining casing was pulled and the cement was topped off and did not drop. Good circulation was established prior to setting all plugs.
0668801545 2015-02-10 Production Wells No Violations Compliance Notice Follow-up -1163157469: According to the operator in a statement, the well is providing house gas. Upon inspection, I found the valve open on the annulus which was connected to a 1 orange plastic line. The operator is planning on plugging the well in the spring.
1163157469 2014-01-23 Production Wells Violations Noted The well is currently producing house gas through a 1 line attached to the 4.5 casing. The tubing is open at the wellhead however it is shut just before the separator. There is no chart on the meter. There is a 100 bbl tank and a 1550 gallon plastic tank in the dike. Operator has reported zero production for 2011 and 2012. There was no production reported from 2008-2010. Operator must plug or produce the well within 90 days.

1698639583 2016-12-07 Final Restoration No Violations This well is currently a producing well in the RBDMS system. Inspector Jake Glasscock sent me an e-mail letting me know this well had been plugged previously. There is a report from Inspector LJ Parkins in the system with the plugging specifications but there is no plugging report from him or Alliance Petroleum Corporation. I contacted Marty from Alliance letting him know this issue and asked him to please send me his plugging report as well as send it to Columbus for our records. The inspection of the site revealed that this well has been plugged and all equipment has been removed and vegetation has been restored. This well is in compliance with 1509.072(B) and final restoration has been passed. I will make another report when I receive the paperwork from Marty.
1894889086 2014-07-07 Production Wells No Violations Compliance Notice Follow-up -1163157469: The well is still idle. The landowner, Sheila Todd, stated that the well has not produced since 2006 and no one is getting house gas off of the well. She stated Alliance came out and worked on the well a while back and were not able to get the well back into production. She stated that she calls the operator a couple of times a year because she wants the well off her property. I talked to Marty Miller of Alliance and he stated he will get a permit to plug in right away and plans on plugging the well in the late summer.
