0103051304 2018-01-09 Production Wells No Violations The operator began flush and vac operations using Delco water heated to 200 degrees and flushed with 4 hoses. Product was flushed and squeegeed on the frozen ground into the retention pits for removal. 30-35 bbls product removed. Total product removed: estimated 140 barrels (water from flush/snow melt, crude, brine and dirt). As a safety precaution two booms were set at the field tile exists, .8 miles West of the location. Shut down for the day.
0211276621 2018-01-07 Production Wells Violations Noted Fishburn responded to the site with a vac truck and dozer and began vacuuming the congealed (due to single digit temperatures) crude oil. Failure of a valve between the storage tanks released crude oil and production water over the containment and into the adjacent crop field (est. 100-120 bbl.). The visible impacted area is approximated at 100L x 30W, the extent of which was limited by the single digit temperatures, but hampered initial clean-up as the vac truck diesel fuel gelled off after an hour and a half. The operator bladed the product toward the storage and contained it with 21 straw bales and saw dust. Notified landowner of clean-up effort and shut down for the day.

The cropland (beans) behind the storage shows stunted and areas of no growth. The area was previously remediated (soil replaced) and showed growth at the beginning of spring. Met with the landowner Mrs. Webb on location and discussed. She had contacted the operator (Fishburn) and is working out a crop damage claim. Any further remediation will be directed between her and Mr. Fishburn. Advised that she contact me if she needed any additional assistance. Contacted the operator to remove two booms set at the field tile exit from January. Relayed information to Emergency Operations Coordinator Ms. Vikki Bunting.

The 250 barrel horizontal brine tank leaked (from an unseen hole) and released brine over the dike wall which then followed a unvegetated drainage courseway within the farmers field. Estimate of release is between 50 to 100 barrels as per operator. No crude oil was involved. Met with the farmers on location (Shoemaker and Webb) and discussed the areal extend and methods of remediation. The field was recently harvested of soybeans and the farmer plans to plant soybean next season. Mapped the lateral extend of the brine release using Quantabs, GPS locations and photos. Status: Will meet with the opearator to discus remediation.

Status: documented crude oil tank over run. According to the operator, during the cold snap the brine line froze: causing the oil tank to overflow (over the weekend). In addition, high winds blew oil outside the storage dike. The operator shut-in the well to wait on weather. On 2-5-19 with warmer weather/snow melt the operator vacuumed the storage dike and put absorbent pads on the oil outside of the storage. At the time-of-inspection the operator responded with a vac truck/hot oil truck and additional absorbent pads. Washed the storage dike and vacuumed product and placed pads on oil outside the containment (south and south/east side). One 12 foot boom was placed across a drainage way south of the release for containment during heavy rains.
