0463034109 2006-09-27 Production Wells No Violations Mike Murnane called and reported a flow line leak, discovered this date by Jack Harris. The spill site had been excavated and the source of the leak fixed by a line patch. Approx 3 to 5 gal of oil has been lost. I advised Jack to rake out the vegetation (dry leaves) that had oil on them also the rock and dirt that came from the hole he dug around the flow line. Then put it on a near by concrete pad. He would like to leave the excavation open for a few days to make sure the patch holds.

1144165727 2006-04-04 Production Wells No Violations On site as the result of the report of an oil spill. Morrow Co EMA Joe Edwards called 4-3-06 in the pm and left a message. I talked to Mike Murnane at approx 11:00 AM on 4/4/06. On the site at approx 11:30 AM. Apperently one of the storage tnaks had a hole in it and when the well was put back to pumping after servicing. The tank leaked and the high winds blew the oil out across the drive to the north and into a pasture field. Some clean up has been started but no one is at the site at this time.

Approx 12:00PM a back hoe arived on site along with Mike Daltion of the OEPA. Clean up started with removing the contaminated material in front of the tanks. Mike Murnane arived on site and we discused, clean up and putting a dike around the storage facilities. I also told Mike that he needs to post the proper Identification at the site.

The storage consists of two tanks and a plastic 1000 gallon brine tank set in a gravel lined dike. The well identification and emergency contact information is marked on tank #149132.
