1246938179 2016-10-25 Production Wells Violations Noted Received notification from the operator of a release at the flowline near the well head. Lost approximately one day of production. Product pooled near the well head and flowed down grade following an existing drainage gully to the woods south of location. Emergency Operations Coordinator Vikki Bunting is on location and remediation is being directed through her. Initially the operator spread saw dust (with biologic material) heavily on the release and allowed time to absorb. Secondly, the downslope soil was removed and placed on a pit liner, covered and surrounded by a earthen dike to prevent run off.

On site at 2:20pm found oil on the ground around the well head approx 5 to 8 gal most likely from stuffing box leaks over the winter. Called Jack Fishburn and told him of the problem, he told me that he would send some one out to clean up the site. Called Mr Hilty and told him what I found and that I have been in contact with Jack Fishburn. I plan on returning to the site on 3/25/11 for a follow up.

Estimated product released is 1.5 days of production as per the operator. The release traveled down a low spot in a freshly planted bean field and pooled up at the fields edge. The well is shut-in and the operator has spread several loads of saw dust on the pooled up areas, added absorbent pads and hay bales to mitigate additional flow. Asked the operator to contact the landowner. Status: contained, remediation started.
