Gary Smith has been sent the packets but is failing to get registered with the well with State Of Ohio. He has been using the gas for the Forge.
0695625934 2017-04-20 Production Wells No Violations The purpose of my inspection is due to a follow-up to Compliance Notice number 1151932649. Mike Corp had a crew out cleaning up the tank site on PN 4119. The tank was moved and the contaminated soil and limestone was taken out down to the clay bed. The dump trailer was loaded with approximately 6-8 ton and hauled down to Kilbarger Landfill by Gary Smith. Gary called and stated that a second truck will be needed and Corp called and had them on their way from Logan, Ohio the second load would be hauled out with a dump truck from Logan, Ohio. The amount that was loaded was approximately 10-16 ton. The #57 limestone was placed on the clay and then 4-5 ton of sand was placed on the gravel to keep from corroding the tank. The Tank was set and plumbed back in and the Impoundment was closed back in. The identification was attached to a pole and placed next to the tank. The bull plugs were installed at the well head. This will resolve the Compliance Notice number 1151932649. I will check back to make sure the vegetation has been established.

1105840500 2014-01-22 Production Wells Violations Noted The well is now owned by S&S Spring shop and I have worked with both parties trying to get transferred. The Compliance Notice is to advise the owner of record with the State is responsible for transfer in order to have well removed from their Bond. Transfer well or Failure to comply may result in further enforcement action.
1151932649 2013-03-21 Production Wells Violations Noted Well is producing well with a pumping unit,4.5x2.375well head. There is a 100 barrel tank that has a dike around it. There is oil inside the dike. There is a brine scald area on the outside of the dike approximately 36x40south of the dike. The tank needs plugs on the valves and the top of the tank. There is holes on tank itself that need patched. The well is still registered in Hopco and not current owner. Current owner is Gary Smith. I talked to Ty Hutchinson(shop foreman) and explained that no product is to pumped in the tank until it is repaired. Remove the scald area soil and replace with top soil. Remove all oil soaked soil in the dike and dispose of. Re build the dike walls around the tank battery and reseed and mulch in. Place identification with current owner name and contact information. Immediately stop pumping into tank. Haul any contents out and get tank fixed. Remove all lines and place plugs in at the valves. You have 10 days to comply with the removal of the soil and replacement with good top soil. Remediation of the soil with lime is acceptable as long as it is placed on another area of the property. I need to be present on all removal and replacement of soil. All work needs to be completed by 4-21-13. Failure to comply may result in legal enforcement action. Fines could be imposed.

The tank was run over by an employee of S&S Spring shop. The pumping unit is still broke in the gear reduction box. I met with Tia Smith the daughter of Gary and Roxy Smith and explained the importance of getting the oil cleaned up. I talked to Gary Smith on the phone and he will contact Mike Corp on the clean-up. I stated to Gary that this oil mess needs to be cleaned up by 4-21-17. Failure to clean up will be handled with emergency clean-up crew. The Roxgar Company will be responsible for the payment.
