0091371358 2016-09-16 Production Wells Violations Noted Approximately 10:00am I arrived at the 119-28121 Copa well #1 site to get an updated inspection. This was a Civil Action Follow-up to Temporary Restraining Order issued in the Trumbull County Court of Common Pleas and the Chiefs Order #2015-535, Chiefs Order #2016-105.

- Failure to respond, cleanup or obtain a contractor to meet the below guidelines will result in the Division hiring a contractor to meet the cleanup standards. Clean up the stream to EPA standards and direction. Clean the land by removing all ponding or loose oil, brine contaminant and all saturated soil; once the soils are removed then they must be disposed of at an approved EPA landfill. Provide a copy of all manifests to Inspector Patrick Shreve.

0303239474 2016-01-07 Production Wells No Violations This is a follow -up inspection to # -1706902567 on the producing well which is owned by Big Sky Energy. Todd Miller noted violations and there were additional issues. 119-28121 well is capable of production. Well is a Rose Run well equipped with a large pumping unit. The well was shut in at the tubing head. The gas was pushing a yellow paraffin wax out of the well at the packing glands. There was a large paraffin mess approximately 4 x 20 along the pumping unit. No material got into the nearby stream. The company was notified and had brought equipment out to start cleanup and containment. The tanks are shared with well 22568. There is 1-210 barrel and 1-100 barrel steel tank in a dike. There is scald on the backside of the dike and within the dike. There are identifications at well and at tanks. Pictures were taken and stored on file. The well coordinates are 40.07701,-81.81197 epe 8. Tank coordinates are 40.08175,-81.81172 epe13. Entry coordinates are 40.08070,-81.81403 epe8.

They had constructed a dike around three sides of the pumping unit and placed a secondary dike to catch the overflow. I will check on the site as the weather warms up.

Nate Lane of the Emergency Response of the Division of Oil and Gas and I went into the site and took pictures. The company had just placed peat absorbent on the wet material lying and did not clean up the material. I called the company Mr. Bob Barr thinks it is coming from the bottom of the unit. He stated that he will remove material with a shovel.

He had skid steer at tanks and had cleaned up the small brine scald at the back of the tank site. There is some small oil spots in the earthen dike that need cleaned up with a shovel. Down by the well the oil is still purging out of the soil and is being trapped by absorbent pads. The company has been installing bio-sorbent on the whole perimeter. The company has been changing pads out. I will check back on 1-2 weeks.

1335285683 2016-01-08 Production Wells No Violations On day of inspection Greg Henderson with Company was bringing in more equipment to clean up oily paraffin. Water truck was brought in to suck up the material which was a slow process. Rain had moved in by this time causing a slow- down. Toward the end of the day the water truck was left on location along with the dozer and the rubber- track hoe. Bags of HTP were placed at the site to aid in the clean-up.
1374373923 2018-10-18 Production Wells No Violations The inspection is conducted as a Chiefs Order follow-up to number 2017-258. The well is equipped with a pumping unit, electric motor with no electric service, tubing head, 2 3/8-inch tubing. The one and two- inch valves are closed. The well head is still burping oil out and causing issues. The is still an area of 4 foot by 6 feet by 10-foot long. The quantabs were ran and did not register any chlorides. The tank site consists of [1]210-barrel steel tank and [1]100-barrel steel tank. This equipment is enclosed within an earthen impoundment. There is a horizontal separator that is consumed with weeds but is not in operation due to the well being shut in.
1474050459 2016-09-16 Administrative Inspection No Violations At 12:52 PM, I called the Ashtabula County Inspector, Jerry Kohl, for the Division and asked him to relay to the well owner, Mr. Bob Barr, that he had an oil spill at this well location and to contact, Inspector Pat Shreve. Note: On this day, Jerry was plugging a well with Mr. Barr in Ashtabula County. I received a phone call from Inspector Shreve at 2:26 PM stating that he had received a text message response from Mr. Barr. At 2:35 PM, I called the well owner, Mr. Bob Barr, at 740-260-
1474129619 2016-09-17 Production Wells No Violations On the day of my inspection I met with Big Sky owner and representatives concerning the oil spill at the well which resulted in oil reaching a tributary of White Eyes Creek. Mr. Barr was in the process of applying absorbent pads to the contaminated area. Discussion was held on the removal of the contamination and disposing of in a designated landfill. Mr. Barr was having trouble obtaining a water truck for extraction of the contamination in the creek and left location to try to find one. Adjoining land owner Bob Glosser arrived and stated that the access road was not being utilized and someone drove through their food plot. I explained to Mr. Glosser what occurred and no one has driven past the well today. I also stated that equipment would stay on the access road during the clean-up process. At 12:08 I placed a call to Mr. Barr and I stated that placing the contamination onto containment and covering up until the first of the week would be sufficient until he could make arrangements to haul to the landfill. Mr. Barr stated that he would not be doing that and we could get someone else to do the clean-up. I placed calls to several operators putting them on standby for clean-up. At 15:30 Mr. Barr reported back to location and began cleaning up oil by hand and with buckets and shovels placing the contamination into 30 gallon polymer barrels. Mr. Barr stated he was unable to find a vacuum truck and requested that I try. I made several calls with no success at getting a vacuum truck on scene. I was able to line up an independent contractor which is on standby for Sunday 9/18/16 at 09:30. I instructed Mr. Barr that the stream needed to be cleaned to EPA standards and the contamination needs to be removed and disposed of in an approved landfill. Mr. Barr stated that he was cleaning the stream and would not be removing the soils. At 16:30 ODNR Emergency Operations Coordinator Vikki Bunting and Southeast/Southwest Emergency Regional Manager Trish Cartmell arrived and were ad
1474279091 2016-09-18 Production Wells No Violations Arrived at 9:30am on site with King excavating and then went in to the site. Vicky Bunting was on site supervising Barr and one guy in the creek extracting the oil with Absorbent pads and wringing them out into the 2-poly barrels of 30 gallons each. They had no other equipment. King brought one loaded water truck with fresh water to purge and one water truck empty to haul. King also brought two dump trucks, dozer and track-hoe. The decision was made to lay the plastic out and place the oil saturated soils on the plastic and cover with plastic when finished. They extracted what fluids they could and then brought track-hoe in and dug area in creek, around pumping unit, well and to the creek. The impoundments were removed and the dirt was smoothed out making a water way around the well site for water to leave the location. King will come back and remove soils when paperwork gets approved at a Columbus landfill approved for EPA. There were approximately 40-50 tons of soils on plastic. The water truck with the water was purging the creek and the remaining oil was picked up with absorbent pads. Vickie installed one more dam in the creek with absorbent booms and pads.
1474297074 2016-09-17 Administrative Inspection No Violations I had received notification from Inspector Doug Bennett that the well owner, Mr. Bob Barr, left the location to find a water truck and stated during a phone conversation that he would not remove the soil impacted from this oil spill and haul to an EPA approved landfill. Mr. Barr further stated to the inspector that the Division could get someone else to perform the cleanup of this spill. At 12:50 PM, I called the well owner, Mr. Bob Barr at 740-260-2154, and left a voicemail message stating for him to confirm with me that he would not be completing the oil spill cleanup. Furthermore, I stated that the soil would need to be removed and hauled to an EPA approved landfill. Mr. Barr would later respond via text message and disagreed with the removal. Mr. Barr was sent a text message that stated the following: You are the well owner/operator and this is an emergency situation that requires your immediate attention. Please have a cleanup crew at this location by 4 pm or a contractor will be mobilized to perform cleanup of land and water. Please be advised that the Division will seek reimbursement for this cost. The soils require removal and proper disposal to a landfill per work order from the Chief.

The well is shut in due to a Temporary Restraining Order issued in the Trumbull County Court of Common Pleas and the Chiefs Order #2015-535, Chiefs Order #2016-105.

The well site had been cleanup by an Emergency contractor and this inspection verified the site at the pumping unit was clean and vegetated back. The creek was clean and vegetated back along the sides of the creek. The well is equipped with a Morgan pumping unit with an electric motor and the tubing head. The valves are all shut at the well. The well is shared with tank site and PN 2568. The tank site consists of {1}210 barrel steel tank, {1}-100 barrel steel tank and a horizontal separator with all valves shut. The dike has been repaired and cleaned up. The identification is posted at the well and the tank site.

The well is equipped with a large pumping unit. The Rod annulus on the production manifold is open and capable of sales. The well is complete with rods and tubing. The prime mover is supplied with an electric motor. The well goes with the shared tank farm on Matchett road. There is 1-210 barrel steel tank and 1-100 barrel steel tank. The dike has been repaired and cleaned up. The 210 tank needs a bull plug at the bottom. There is a horizontal separator that the valves are cracked and in use. The wells could be selling into Columbia natural gas line. This well has been on an ongoing cleanup since the last inspection.

2045950649 2018-04-27 Production Wells Violations Noted The conditions are dry and clear. The temperature is 76 degrees Fahrenheit. The inspection was conducted as a complaint filed under number 5826. The inspection revealed that the stuffing box had released and let the oil blow with an area of 6 feet by 30 feet. I called Bob Barr at 14:49 pm. I stated that I need the oil contaminated area cleaned up and hauled away. (Mr.) Barr stated that he thought the crude oil was coming from the bottom of the unit. I stated I need the area cleaned up in 24 hours. He responded promptly and called back at 16:00pm and stated that he placed Pete - Absorb (a type of mulch to absorb oil) on the affected area. He would get affected area cleaned up as soon as Greg Henderson fixed the small dump truck.
