0264095406 2015-11-10 Production Wells No Violations On the day of my inspection I witnessed the production equipment included a 100bbl tank within an earthen dike and a 2 inch sales meter with .25 inch orifice and a chart recorder. The well was equipped with 8.625 inch casing, 4.5 inch casing, 1.5 inch tubing, rods, a Churchill pumping unit powered by natural gas and a drip with valves open on the casing and 1 of 2 valves open on the tubing. Identification was present and production records were current with the access road meeting ORC 1509.072A.

1557319594 2019-05-08 Plug / Plug Back No Violations On site inspection to witness the setting of the bottom hole plug and the plug to isolate the top of the Lime. On the previous day perforations were shot in the 4.5 production casing by Timcos Wireline service between the depths of 4,009 and 4,019. The 1.900 tubing string was then ran back in the hole to a depth of 5,345. On this day Formation Cementing set a 7.3 barrel mix bottom hole plug consisting of 35 sacks of class A cement weighing 15.6 pound per gallon by weight of scale. The plug was displaced with 12.3 barrels of brine water. The plug was set between the depths of 5,345 and 4,895 with circulation to surface throughout the pumping of the cement plug. Following the bottom hole plug the 1.900 tubing was pulled up to a depth of 4,054 and a 300 plug was set to cover the top of the Lime. The plug was 18 barrels total volume consisting of 86 sacks of class A cement with calcium chloride only as an additive. The plug was displaced with 9.3 barrels of brine water with circulation established and maintained through out the pumping of the plug. The well did circulated behind the 4.5 during the job indicating circulation between the 8.625 and 4.5 diameter casing strings. Proceeding the pumping of the Lime isolation plug the 1.900 tubing was pulled out of the hole and the plug tagged at 3/737 before ripping the casing at 3,619. The casing will be pulled out of the hole and the 1.900 tubing ran back in the hole to set plugs across the bottom of the surface casing and the surface plug.
1557498267 2019-05-10 Plug / Plug Back No Violations On site inspection to witness the completion of the well plugging with a service rig on the hole owned by Bishop Well Service and Formation Cementing. The Company man on site is Steve Minor with Artex. The 4.5 diameter casing was ripped at 3,619 above the top of the lime plug tagged at 3,737. The 4.5 diameter casing was recovered from 3,619 and 1.900 tubing ran back in the hole to a depth of 1,771. A 9.2 barrel class A plug was set to cover the bottom of the 8.625 surface casing. The plug consisted of 61 sacks of cement with 100 pounds of calcium and cotton seed hulls as additives. The displacement was 3.9 barrels of fresh water. The tubing was pulled up to 225 and a final surface plug set, the plug was a 15 barrel plug consisting of 71 sacks of class A cement with calcium chloride and cotton seed hulls as additives. Cement was witnessed to surface, the tubing was pulled and the well topped off to surface. Cement stayed up to surface. The plugging process was complete on this day.
2118477681 2019-05-07 Plug / Plug Back No Violations On site inspection to witness the perforation of the 4.5 diameter casing string with a service rig on the hole owned by Bishop Well Service and Timcos Wirelne Service. The 1.900 diameter productin tubing was pulled out of the hole and the 4.5 diameter casign was perforated 200 below the top of the lime, the top of the Lime is at 3,819 and the well was perforated betweent he depths of 4,009 and 4,019. There was 10 perforations shot.
