1434966379 2015-07-23 Plug / Plug Back No Violations I received a phone call from Chip Smith, owner of Chipco, indicating that a service rig was moving onto location. Upon arrival I found Chipco rig four working the 1.5 tubing. The rig crew ran sand line tools to a depth of 5180 and could not get any deeper. Circulation could not established The decision was made to cut the tubing. Appalachian Well Surveys cut the tubing at 5180.
1438102768 2015-07-27 Plug / Plug Back No Violations I am onsite to witness plugging operations. Greenes Welding installed a hot tap on the wellhead to allow the pressure to be released. Chipco pumped a 60 sack 15.6 pound per gallon class A cement plug through 1.5 tubing from 4370 to 4170. The service rig crew then pulled the tubing out of the well. A casing ripper was utilized to cut the 4.5 casing at the first point of free pipe at 3102.
1445302735 2015-07-28 Plug / Plug Back No Violations I am onsite to witness plugging operations. Chipco pumped a 200 60 sack class A cement plug through the 4.5 casing at 3102. Circulation was gained and approximately 90 barrels of oil was circulated out of the hole. The casing was pulled up to 1859 and a 200 class A plug was placed through 4.5 casing. The casing was pulled up to 200 from surface and a class A plug was circulated back to surface. The cement stayed at the surface. Chipco was instructed to reclaim the site in accordance with 1509.072(B).
1445927518 2015-10-05 Final Restoration No Violations My inspection today finds the well site meets the final restoration requirements of 1509.072(B).
1711842866 2015-07-24 Plug / Plug Back No Violations Chipco set a 30 sack 15.6 pound per gallon cement plug at 5180 through 1.5 tubing. The service rig crew pulled the tubing out of the well and Appalachian Well Surveys perforated the casing at 4340-4360 100 below the top of the Big Lime with 20 shots in 20.
