The pipeline tie in is located on the north side of the pad. The production records are current and the identification is located at the wellhead.

The production valves are open. The production equipment consists of five gas processing units, eight 400 barrel tanks within containment, and two flare stacks. The production records are current, identification is located on the wellhead, and there are no violations present.
0565116061 2012-11-11 Drill / Deepen / Reopen No Violations My inspection today finds the well operator is having a bond log performed by RWS. After bond logging they determined and I concurred that perforating at 675 would be a good point to perforate and attempt to squeeze cement. Allied attempted to circulate, but the well began to pressure up after 1.5 bbl. And did not circulate. The rig crew is again running the 1 tubing to a depth of 350. Cemented back to surface. A total of 75 sks of cement were used.

The pipeline tie in is located on the north side of the pad. The production records are current, the identification is located at the wellhead and gas production unit, and there are no violations present.
0655665267 2018-11-01 Production Wells No Violations On the day of my inspection I witnessed that the well was equipped with 5.5-inch diameter production casing and 2.375-inch diameter tubing equipped with a plunger lift system. At the time of my inspection annular pressure monitoring was in place, and the 13.375-inch diameter casing had 450 pounds per square inch, the 9.625-inch diameter casing had 225 pounds per square inch. Production equipment included a Gas Processing Unit, as well as 8 400-barrel steel production fluid tanks, and 2 combustors that were shared with the other producing wells on the pad. There was midstream equipment on the Northern Corner of the pad. There was secondary containment around the production fluid tanks. The well pad was contained within an earthen dike, there were multiple Emergency Shutoff Devices on location. Identification tags were present, and production was up to date.

1352256419 2012-11-05 Drill / Deepen / Reopen No Violations I am on site to witness the cementing of the 13.375 casing. The casing string is ran to a depth of 1847. Allied Well Service is on site to pump the cement. At 37 bbl of displacement the pressure jumped to 1600 psi, and continued to climb to 2000 psi. Total cement displaced was 71 bbl. I instructed the company man Delph Martinez to drill out the cement in the pipe, and prepare to find the cement top on the back side of the pipe.
1352392200 2012-11-08 Drill / Deepen / Reopen No Violations I received a call from Delf Martinez of Antero stating that the rig crew had drilled out the cement, and they were ready to pull a bond log. When I arrived the rig crew was having trouble pulling the pipe. RWS wire line services bond logged the well from 1800 to surface. The cement top was at 1160 from surface. The rig crew attempted to run 1 tubing down the back side of the 13.375 tubing to facilitate cementing. The pipe stopped at 350 from surface.
1352679466 2012-11-09 Drill / Deepen / Reopen No Violations I received a call from Delf with Antero, stating that they would be ready to attempt to cement the surface string in an hour. Upon arrival I found the perforators waiting on equipment. I spoke to Delf about their plans. They intended to perforate again and try to squeeze cement again.

1377601991 2013-08-25 Production Wells No Violations At 5:37 Sam Meyers called and stated that there had been a spill at the site. My inspection found that condensate was released onto the ground outside of containment. The condensate ran down the sites main drainage ditch and appears to have stopped approximately 40 from Opossum Run. Meyers Trucking is onsite to clean and contain. I estimate 5 bbl of contaminate was released.

Production valves were in the open position. A gauge was present on the production tubing reading 260 pounds per square inch, 280 pounds per square inch on the production casing, 0 pounds per square inch on the 9.625 inch diameter intermediate casing, and 0 pounds per square inch on the 13.375 inch diameter surface casing. The production equipment consisted of eight 400 barrel tanks, five gas production units, two combustors, and a Triethylene Glycol Dehydration Unit. The Triethylene Glycol Dehydration Unit had an inlet separator, a Glycol contact tower, a horizontal scrubber, and a reboiler. The identification was located at the wellhead.

Hull Environmental is testing the soil to find the limits of contamination. Ryan Environmental will begin removing soil once the limits have been found.
