0616226791 2017-08-07 Production Wells No Violations The purpose of my inspection today was a status check on a production well. The pad was named Ervin and it consisted of 8 wells. During my inspection the well was equipped with a plunger lift system with production valves in the open position. The production casing had a gauge present reading 300 pounds per square inch and the production tubing had a gauge present reading 182 pounds per square inch. The production equipment consisted of fourteen 400 barrel tanks, 8 gas production units, 4 compressors, and 2 combustors. The natural gas pipeline tie in was located on the south side of the pad. The identification was located on the well and the 400 barrel tank.
0878748438 2015-08-03 Production Wells No Violations My inspection today finds the well is equipped in a manner consistant with a being capable of production. The well is produced into a tank battery comprised of four 400 barrel tanks set inside of a plastic lined steel containment. The production tanks for this well are not set with the tanks for the other wells on the pad.
1056958754 2015-07-10 Production Wells No Violations Status check inspection as a follow up to a on site accident that occurred on 07-08-2015. Upon inspection on this day I met with Anteros HSE representative Chris Warnock and Myers Well Service Regional Safety officer John Moore who indicated the employee of Myers Well Service was injured when a 3 line used in the production fluid transfer process was attempted to be disconnected from the water truck while under pressure causing the line to release from its connection and struck the employee in the face causing lacerations and fractures to the face. The line was free from fluid and no well bore related product was released. The company(Antero) has implemented a safety stand down until the incident can be investigated and corrections can be made to further avoid any future incidents. Addressing the well location a total of 8 wells were found on site and in production with a total of 14 production tanks present with a total capacity of 400 barrels each. All tanks were found inside of a poly lined metal construction containment dike area. Also on site there is a total of 8 gas processing units one supporting each existing well on the location. With proper well identification posted at the well head. Myers Well Service Ohio UIC # 25-427 water trucks were on location along with Ohio Oil Gathering hauling production fluid from the site.

1930031610 2016-01-13 Production Wells No Violations My inspection today finds the well is equipped in a manner consistent with being capable of production. The tank battery is comprised of fourteen 400 barrel tanks set inside of a plastic lined, liquid tight steel containment system. There are four incinerators set at the location. Seven other wells are located on the pad and share the production equipment. The ground is covered by three inches of snow at the time of inspection.
1995953362 2017-01-25 Production Wells No Violations The purpose of my inspection is a status check on a producing well. The pad name is Ervin Pad and there are a total of 8 wells on this pad. During my inspection the well was equipped with a production tree style wellhead. The production equipment for this well consists of 14 400 barrel tanks within containment, and eight gas processing units. The vapor recovery system consists of two compressors and four Cimarron combustors. The production records are current, the identification is located at the wellhead, and there are no violations present.
