0176599631 2017-01-11 Production Wells No Violations The purpose of my inspection today is a status check on a producing well. There are six wells on this pad; 3412124408, 3412124409, 3412124407, 3412124471, 3412124472, and 3412124428. During my inspection the well was equipped with a production tree style wellhead. The production valves are open. The production equipment consists of six gas processing units, eighteen 400 barrel tanks within containment, and two flare stacks. The production records are current, identification is located on the wellhead, and there are no violations present.
0397331906 2013-11-22 Administrative Inspection No Violations At 7:48 p.m. I received a call from James Harvey of Antero. Mr. Harvey stated that a Meyers water truck had rolled over and spilled 5 gallons of motor oil and a minimal quantity of flow back water. He also stated that all fluids were cleaned and Hulls Environmental was on site to get soil samples. All contaminated soil will be removed.
0454049273 2013-12-18 Preliminary Restoration No Violations The preliminary restoration requirements have been met.
0677308600 2017-06-07 Production Wells No Violations The purpose of my inspection was a status check on a producing well. The pad is named Rich and there are a total of six wells on this pad. During my inspection the well was equipped with a production tree style wellhead and a plunger lift system with production valves in the open position. Gauges present read 320 pounds per square inch on the casing and 260 pounds per square inch on the tubing. Annular pressure monitoring was present. The production equipment consists of eighteen 400 barrel tanks within a lined steel containment, six gas processing units within a lined steel containment, two compressor, and two combustors. The pipeline tie in is located on the north side of the pad. The production records are current, the identification was located at the wellhead and gas production unit, and there were no violations present.
0737873295 2013-12-18 Preliminary Restoration No Violations The preliminary restoration requirements have been met.
0810187447 2013-07-12 Casing String Witnessed No Violations I am onsite today to witness the running of surface casing. The rig crew ran casing to 516. At that point the casing became stuck and they lost circulation. The rig crew will attempt to wash down 1 tubing along side the casing in order to regain circulation.
0910202958 2013-07-17 Drill / Deepen / Reopen No Violations My inspection today finds the drilling rig is drilling 17.5 hole at a depth of 1111.
0960551752 2015-04-02 Production Wells No Violations I am accompanied Inspector David Ball today. Our inspection finds the well appears capable of production with the production valves open. The tank battery is comprised of eighteen 400 barrel tanks set in a liquid tight plastic lined steel containment. A separate containment area holds five production units. The production equipment is shared with four other wells on the pad. Equipment associated with the recent stimulation activities is being removed from the site.
1287737615 2013-07-08 Drill / Deepen / Reopen No Violations The rig is bing moved to the well and preparing to drill the surface section of the hole to approximately 560
1334675724 2016-03-21 Production Wells No Violations My inspection today finds the well is equipped in a manner consistent with being capable of production with a tubing plunger lift system. The tank battery is comprised of eighteen 400 barrel tanks set in a liquid tight plastic lined steel containment. A separate containment area holds six production units. The production equipment is shared with five other wells on the pad.
1367426765 2013-04-26 Casing String Witnessed No Violations My inspection today finds the driller has made 1 of hole in 4 hours. Due to the hole conditions I have allowed the operator to run 86 of 26 conductor. Superior Well Service cemented the pipe in place with 278 sks of 15.6# cement. The cement stayed at surface.
1462448049 2013-07-25 Drill / Deepen / Reopen No Violations My inspection today finds the drilling rig is drilling 12.25 hole at a depth of 3127. I tested a sample of water located next to the drilling additives containment area, for chlorides. The water did not contain elevated chloride levels. (tested with Quantabs)
1484938976 2019-11-27 0:00 PW No Violations 0.50
1510348739 2017-11-07 Production Wells No Violations The purpose of my inspection was a status check on a production well. The pad is named Rich and there are a total of six wells on this pad. During my inspection the well was equipped with a production tree style wellhead and a plunger lift system with production valves in the open position. The production equipment consisted of eighteen 400 barrel tanks within a lined steel containment, six gas production units within a lined steel containment, one vent scrubber, two compressors, and two combustors. The pipeline tie in is located on the north side of the pad. The production records are current and the identification was located at the wellhead.
1718652464 2018-06-12 Production Wells No Violations The purpose of my inspection was a status check. The well was equipped with a production tree style wellhead with a plunger lift system. Production valves were in the open position. A gauge was present on the production tubing reading 302 pounds per square inch, 350 pounds per square inch on the production casing, 350 pounds per square inch on the 9.625 inch diameter intermediate casing, and 0 pounds per square inch on the 13.375 inch diameter surface casing. The production equipment consisted of eighteen 400 barrel tanks, six gas production units, and two combustors. The identification was located at the wellhead.
176599631 2017-1-11 0:00: PW No Violations 0.50
1775166437 2013-07-14 Casing String Witnessed No Violations My inspection today finds the operator has regained circulation and the current depth of casing is 2 below the permit required depth. Nabors Well Service cemented 492 (ground level) of 18.625 casing in place with 1202 sks of cement. I witnessed 90 bbl of cement returns to surface.
1886765760 2013-07-15 Drill / Deepen / Reopen No Violations My inspection today finds the drilling rig is tripping in the hole and preparing to drill the 17.5 section of the well.
1969287941 2018-11-01 Production Wells No Violations On the day of my inspection I witnessed that the well was equipped with 5.5-inch diameter production casing and 2.875-inch diameter tubing equipped with a plunger lift system. At the time of my inspection annular pressure monitoring was in place, and the 13.375-inch diameter casing had 340 pounds per square inch, the 9.625-inch diameter casing had 0 pounds per square inch. Production equipment included a Gas Processing Unit, as well as 6- 400-barrel steel production fluid tanks, and 2 combustors that were shared with the other producing wells on the pad. There was secondary containment around the production fluid tanks, and the Gas Processing Units. The well pad was contained within an earthen dike, there were multiple Emergency Shutoff Devices on location. Identification tags were present, and production was up to date.
2030301767 2014-03-31 Production Wells No Violations My inspection finds well is in production. The tank battery consists of eighteen 400 bbl tanks that are shared with three other wells. The identification is posted as required. The well site is being prepared for a drilling rig.
