0155478500 2017-10-18 Production Wells No Violations The purpose of my inspection was a status check on a production well. The well pad name is Myron and it consists of 5 wells. The well was equipped with a production tree style wellhead with a plunger lift system. Production valves were in the open position. Annular pressure monitoring was present. The gauge on the production casing read 330 pounds per square inch and the gauge on the production tubing read 230 pounds per square inch. The production equipment consisted of eight 400 barrel tanks within steel line containment and five gas production units. A vapor recovery system consisted of two compressors, one vent scrubber, and two combustors. A dust suppressant had been recently applied to the lease road. The identification was located at the wellhead and the production records were current.

0707735855 2018-11-26 Production Wells No Violations On the day of my inspection I witnessed that the well was equipped with 5.5-inch diameter production casing and 2.375-inch diameter tubing equipped with a plunger lift system. At the time of my inspection annular pressure monitoring was in place, and the 13.375-inch diameter casing had 140 pounds per square inch, the 9.625-inch diameter casing had 0 pounds per square inch. Production equipment included a Gas Processing Unit, as well as 8- 400-barrel steel production fluid tanks, and 2 combustors that were shared with the other producing wells on the pad. There was secondary containment around the production fluid tanks, and the Gas Processing Units. The Gas Processing Units and midstream valve site were on a secondary pad to the southeast of the well pad. The well pad was contained within an earthen dike, there were multiple Emergency Shutoff Devices on location. Identification tags were present, and production was up to date.

Deep Well Services were on location completing a material modification alongside of the lease road.
