0574541030 2015-02-11 Preliminary Restoration No Violations My inspection today finds the well site meets the preliminary restoration requirements. The tank battery is comprised of eight 500 barrel tanks set inside a liquid tight plastic liner.
0610280216 2018-03-08 Production Wells No Violations The purpose of my inspection was a status check on a cut slope slip failure. The pad name is Roe and there are a total of 7 wells. The slip occurred in the cut slope of the pad in the Northwest corner where the eight 400 barrel tanks and two vapor recovery towers were located. That equipment had been removed in order for Antero to take remedial action on the slope failure. The remaining production equipment and wellheads were outside the area of concern. ODNR Division of Oil and Gas Engineering Section is currently working with Antero Resources with planning the material modification to repair the slope.
0683636919 2017-01-20 Production Wells No Violations The purpose of my inspection is a status check on a producing well. The pad name is Roe and there are a total of 7 wells on this pad. During my inspection the well was equipped with a production tree style wellhead. The production equipment for this well consists of eight 400 barrel tanks within containment, and seven gas processing units. The vapor recovery system consists of two compressors and three Cimarron combustors. The production records are current, the identification is located at the wellhead, and there are no violations present.
0767192811 2013-08-28 Casing String Witnessed No Violations On site to witness cementing of conductor casing, Allied cementing pumped336 sacks of cement and they had good returns and they left 2.5 inside of bottom of cellar to ensure that there will be no ground water contamination.
0836444505 2018-04-20 Production Wells No Violations During my inspection contractors for Antero Resources were on location running lines to the combustors. The production equipment had been relocated due to a slip near the old orientation of the production equipment on the north side of the pad.
0962093222 2015-06-16 Production Wells No Violations The well is equipped in a manner consistent with being capable of production. All drainage sumps on the location are closed. No violations were noted during the inspection.
0994101880 2014-04-30 Production Wells No Violations My inspection finds the diesel spill that took place during the completion phase is still causing some sheen on top of the water in the sump. The company is hauling all water off site to be disposed of or recycled. I have asked the company to remove all contaminated stone and soil. The tank battery is comprised of eight 400 bbl tanks set in a containment system.
0994415861 2018-07-16 Production Wells No Violations The purpose of my inspection today was to witness Antero re-assembling the Grissom 1H plunger lift system. The well was equipped with a plunger lift system with valves in the open position. A pressure gauge was present on all casing strings. Accompanied by Antero Production Superintendent during my inspection.
1078012218 2013-11-27 Drill / Deepen / Reopen No Violations My inspection today finds the rig has completed the BOP pressure test. They are running drill pipe in the hole to begin drilling the 8.5 section.
1129304378 2020-4-28 0:00: PW No Violations 0.20
1171915293 2013-12-03 Casing String Witnessed No Violations My inspection today finds the drilling rig is running 5.5 casing at a depth of 7888. Allied will be cementing the casing in place later tonight.
1280317832 2018-03-21 Production Wells No Violations The purpose of my inspection was a status check. The pad is named Roe and there are a total of 7 wells. During my inspection construction equipment was on location to make repairs to the slips located on the cut slope and fill slopes of the pad. Antero is actively working with ODNR Division of Oil and Gas Engineering section to make necessary slip repairs. The slips do not oppose any threat to the wells and the wells are under continuous pressure monitoring.
1383316000 2013-10-31 Drill / Deepen / Reopen No Violations My inspection today finds Patterson Drilling rig 342 is currently drilling 12.25 hole at 6352.

1486321491 2013-10-21 Casing String Witnessed No Violations My inspection today finds the drilling rig, Patterson 342, ran 388 of 18.625 casing. Allied cemented the casing in place with class A cement.
1630127591 2017-05-09 Production Wells No Violations The purpose of my inspection was a status check on a producing well. The pad is named Roe and there are a total of 7 wells on this pad. During my inspection the well was equipped with a production tree style wellhead with valves in the open position. Gauges present read 310 pounds per square inch on the casing and 170 pounds per square inch on the tubing. Annular pressure monitoring was present. The production equipment for this well consists of eight 400 barrel tanks, seven gas processing units, two compressors, and two Cimarron combustors. The pipeline tie in is located on the west side of the pad. The production records are current, the identification is located at the wellhead, and there are no violations present.
1710542102 2013-10-21 Drill / Deepen / Reopen No Violations My inspection today finds the rig has drilled to 380 and is preparing to pull the drill pipe. The permits are posted and safety equipment is present.
1730508642 2014-05-16 Production Wells No Violations The well is equipped to be capable of production. The well is produced into eight 400 bbl tanks set in liquid tight containment. The GPS data was updated to enhance accuracy.
