I could find no water to sample on the out slope. The catch basin is empty. Sam Meyers with Antero EH&S stated that the rain water sumps on location have been cleaned and a couple of cracks have been repaired.
0709397267 2017-10-18 Production Wells No Violations The purpose of my inspection was a status check on a production well. The well pad name is Cynthia and it consists of 5 wells. The well was equipped with a production tree style wellhead with a plunger lift system. Production valves were in the open position. Annular pressure monitoring was present. The gauge on the production casing read 280 pounds per square inch and the gauge on the production tubing read 170 pounds per square inch. The production equipment consisted of eight 400 barrel tanks within steel line containment and five gas production units. A vapor recovery system consisted of two compressors, one vent scrubber, and three combustors. The pipeline tie in was located on the North side of the pad. A dust suppressant had been recently applied to the lease road. The identification was located at the wellhead and the production records were current.
0788673782 2015-07-05 Production Wells No Violations My inspection today is in response to the condensate release that took place the previous day. My inspection finds there is no visible condensate on the well pad. A light layer of condensate can be seen in the drainage sumps. I spoke to Mr. Goddard with Antero and he stated that they will steam clean the sumps as soon as possible, and he stated that Micro-Blaze bio-remediation product was applied to the affected areas after the contaminated dirt was removed. I requested that Antero provide the manifests for all soil removed from the site, as well as, the disposal tickets for water hauled off of location.

There are three vapor recovery units and three incinerators set at the location. Four other wells are located on the pad and share the production equipment. The ground is covered by three inches of snow at the time of inspection.
1432151640 2015-05-20 Production Wells No Violations At 12:22 pm I received a telephone call from Sam Myers, Antero EH&S. Mr. Myers indicated that an unknown quantity of flow back brine leaked out of roll-off tanks on location and then escaped through a hole in the secondary containment beneath them during a recent large rain event. Upon my arrival on site at 12:33 pm I witnessed two vac trucks removing water from the site and Hull Environmental performing testing of the area. My inspection found that all drainage outlets were closed. I sampled standing water at three location on the well pad and three locations on the out slope of the well pad with a high range Quantab and found levels ranging from 6627 parts per million (PPM) to less than 250 (PPM). Dispose of all accumulated water on the site until chlorides are mitigated. A lined catch basin is to be installed at the base of the affected out slope. Antero shall submit third party testing results as they become available.

A test with low range Quantabs indicated the total chlorides were at 613 ppm. No water was witnessed flowing from the out slope.

1666913524 2015-06-16 Production Wells No Violations My inspection today finds that the chlorides on the location at the North drainage sump are at 3.4 on a high range Quantab indicating total chlorides of 533 parts per million (PPM). A test sample was taken from the catch basin at the base of the out slope, that sample tested at 4.2 on a high range quantab indicating total dissolved chlorides of 755 PPM. A small puddle of standing water on the out slope (ODNR-1) tested at 6.6 with a high range Quantab 1982 PPM. Antero has dispatched water trucks to remove all accumulated water on the location and from the catch basin.

I sampled the water at two points with low range Quantabs and found the total chlorides at a level of 229 ppm. The water in the catch basin at base of the out slope also tested at 5.0 (229 ppm). Todays results resolve the violation against the well.
1899749186 2018-11-27 Production Wells No Violations On the day of my inspection I witnessed that the well was equipped with 5.5-inch diameter production casing and 2.875-inch diameter tubing equipped with a plunger lift system. At the time of my inspection annular pressure monitoring was in place, and the 13.375-inch diameter casing had 450 pounds per square inch, the 9.625-inch diameter casing had 500 pounds per square inch. Production equipment included a Gas Processing Unit, as well as 8- 400-barrel steel production fluid tanks, and 2 combustors that were shared with the other producing wells on the pad. There was secondary containment around the production fluid tanks, and the Gas Processing Units. There was a midstream valve site on the northwest side of the pad. The well pad was contained within an earthen dike, there were multiple Emergency Shutoff Devices on location. Identification tags were present, and production was up to date.
1902641715 2015-07-04 Production Wells No Violations At 1:46 PM I received a telephone call from Aaron Goddard, Antero EH&S Manager, indicating that a 10 barrel condensate release took place on the Cynthia Pad. Mr. Goddard indicated that the product was suspended on rain water that accumulated on the pad. All sumps were closed at the time of the release due to a previous incident.
1930281127 2015-06-09 Production Wells No Violations My inspection today finds the chloride levels at the northern location drain, a small puddle on the out slope of the location and the catch basin at the base of the out slope were all above the Divisions acceptable threshold. I will continue to monitor the site.
1941247898 2015-06-04 Production Wells No Violations This inspection is a follow up to the Compliance Notice issued on 5/20/2015. My inspection today finds that fluid has collected in the catch basin built at the base of the northern out slope. A sample was withdrawn from the basin by a 250 ml poly bottle. The sample was then tested with two high range Quantabs, one from batch 3275 and one from batch 4134. Both yielded a reading between 7.6 and 7.8 indicating total chlorides in excess of 3000 PPM. A puddle of standing water half way down the out slope (39.88194-81.40429) was sampled by withdrawing a fifty ml sample with a syringe. The sample was placed in a 250 ml ply bottle and tested for chlorides. The test with high range Quantabs, again from batch 3275 and 4134, indicated chlorides were present in excess of 6627 PPM. I called and spoke to Sam Myers at 9:08 am. Mr. Myers agreed that the company would continue to monitor the site and haul water from the sumps on location and the catch basin to an approved injection or recycling facility. I agreed with Mr. Myers that the company has performed in a workman fashion and that more time than was originally allotted is necessary to allow for proper remediation. I agreed extended the compliance date to 9/4/2015.
