0296683751 2014-03-05 Casing String Witnessed No Violations My inspection finds the drilling, Patterson 343, ran 2108 of 13.375 casing. Allied cemented the casing in place by circulating 1782 sks of class A cement into place. I witnessed 60 bbl of cement returns to surface.
0484199323 2014-03-01 Casing String Witnessed No Violations My inspection finds the rig ran 385 of 18.625 casing. Allied cemented the casing in place with 782 sks of class A cement. I witnessed 52 bbl of cement returns of cement.
0985554360 2016-07-28 Production Wells No Violations My inspection today finds the well is equipped in a manner consistent with being capable of production. The tank battery consists of eight 400 barrel tanks set in a plastic lined liquid tight steel containment. The tank battery is shared by one other well on the pad. Two vapor recovery compressors and two incinerators are located near the entry of the well pad. Heavy rain is falling at the time of inspection.
1416625693 2018-11-27 Production Wells No Violations On the day of my inspection I witnessed that the well was equipped with 5.5-inch diameter production casing and 2.375-inch diameter tubing equipped with a plunger lift system. At the time of my inspection annular pressure monitoring was in place, and the 13.375-inch diameter casing had 240 pounds per square inch, the 9.625-inch diameter casing had 580 pounds per square inch. Production equipment included a Gas Processing Unit, as well as 8- 400-barrel steel production fluid tanks, and 2 combustors that were shared with the other producing wells on the pad. There was secondary containment around the production fluid tanks, and the Gas Processing Units. There was a midstream valve site on the northwest side of the pad. The well pad was contained within an earthen dike, there were multiple Emergency Shutoff Devices on location. Identification tags were present, and production was up to date.
1445094836 2017-12-01 Production Wells No Violations The purpose of my inspection was a status check on a production well. The well pad name is Smierciak and it consists of 2 wells. The well was equipped with a production tree style wellhead with a plunger lift system. Production valves were in the open position. Pressure monitoring was present on all casing strings. The production equipment consisted of eight 400 barrel tanks within steel line containment and two gas production units. A vapor recovery system consisted of two compressors, one vent scrubber, and two combustors. The pipeline tie in was located on the North side of the pad. Lease road was in good condition with vegetation established alongside the road.
1508411293 2017-10-18 Production Wells No Violations The purpose of my inspection was a status check on a production well. The well pad name is Smierciak and it consists of 2 wells. The well was equipped with a production tree style wellhead with a plunger lift system. Production valves were in the open position. Annular pressure monitoring was present. The gauge on the production casing read 280 pounds per square inch and the gauge on the production tubing read 150 pounds per square inch. The production equipment consisted of eight 400 barrel tanks within steel line containment and two gas production units. A vapor recovery system consisted of two compressors, one vent scrubber, and two combustors. The pipeline tie in was located on the North side of the pad. A dust suppressant had been recently applied to the lease road. The identification was located at the wellhead and the production records were current.
1546734709 2014-03-03 Drill / Deepen / Reopen No Violations I received an email from Antero company representative Rossendo Perez stating that the rig is drilling 17.5 hole at a depth of 1230.
1775537583 2014-03-11 Drill / Deepen / Reopen No Violations My inspection today finds the rig is currently drilling a 12.25 hole at a depth of 4045.
1792270486 2014-07-09 Preliminary Restoration No Violations My inspection today finds the well meets the preliminary restoration requirements. The tank battery consists of eight 400 barrel tanks set in a liquid tight containment and is shared by another well on the pad.
1920485645 2014-03-19 Drill / Deepen / Reopen No Violations My inspection today finds the drilling rig is drilling 8.5 hole at a depth of 12494.
2052163147 2014-01-27 Drill / Deepen / Reopen No Violations My inspection today finds the well has been cemented. The cement is visible at surface.
