0201810431 2017-11-07 Production Wells No Violations The purpose of my inspection was a status check on a production well. The pad is named Rich and there are a total of six wells on this pad. During my inspection the well was equipped with a production tree style wellhead and a plunger lift system with production valves in the open position. The production equipment consisted of eighteen 400 barrel tanks within a lined steel containment, six gas production units within a lined steel containment, one vent scrubber, two compressors, and two combustors. The pipeline tie in is located on the north side of the pad. The production records are current and the identification was located at the wellhead.

During my inspection the well was equipped with a production tree style wellhead and a plunger lift system with production valves in the open position. Gauges present read 420 pounds per square inch on the casing and 230 pounds per square inch on the tubing. Annular pressure monitoring was present. The production equipment consists of eighteen 400 barrel tanks within a lined steel containment, six gas processing units within a lined steel containment, two compressor, and two combustors. The pipeline tie in is located on the north side of the pad. The production records are current, the identification was located at the wellhead and gas production unit, and there were no violations present.
0354725626 2014-03-10 Drill / Deepen / Reopen No Violations I stopped at the rig to pick up the cement tickets for the surface string. I called Darren Rader with Antero and asked him to remove accumulated trash from the perimeter of the site.
0614544866 2018-11-01 Production Wells No Violations On the day of my inspection I witnessed that the well was equipped with 5.5-inch diameter production casing and 2.875-inch diameter tubing equipped with a plunger lift system. At the time of my inspection annular pressure monitoring was in place, and the 13.375-inch diameter casing had 380 pounds per square inch, the 9.625-inch diameter casing had 420 pounds per square inch. Production equipment included a Gas Processing Unit, as well as 6- 400-barrel steel production fluid tanks, and 2 combustors that were shared with the other producing wells on the pad. There was secondary containment around the production fluid tanks, and the Gas Processing Units. The well pad was contained within an earthen dike, there were multiple Emergency Shutoff Devices on location. Identification tags were present, and production was up to date.

The tank battery is comprised of eighteen 400 barrel tanks set in a liquid tight plastic lined steel containment. A separate containment area holds five production units. The production equipment is shared with four other wells on the pad. The well site meets the preliminary restoration requirements.

1115152785 2018-06-12 Preliminary Restoration No Violations The purpose of my inspection was a status check on a preliminary restoration. The well was equipped with a production tree style wellhead with a plunger lift system. Production valves were in the open position. A gauge was present on the production tubing reading 860 pounds per square inch, 867 pounds per square inch on the production casing, 420 pounds per square inch on the 9.625 inch diameter intermediate casing, and 380 pounds per square inch on the 13.375 inch diameter surface casing. The production equipment consisted of eighteen 400 barrel tanks, six gas production units, and two combustors. The identification was located at the wellhead. The area effected by the construction of the pad had been restored with no erosion present and vegetation coverage. The location has met the requirements for preliminary restoration written under 1509.072 (A).

1394376823 2014-03-09 Casing String Witnessed No Violations My inspection finds Falcon Drilling ran 401 of j-55 casing. Allied cementing circulated 848 sks of class A cement with 2% CaCl and .25 pps of Floseal per sk. I witnessed Allied weigh the cement twice with a pressurized cement scale. The first test weighed 15.7 ppg and the second weighed 15.6. There were 39 bbl of cement to surface.
1424303163 2014-11-03 Drill / Deepen / Reopen No Violations On November 2, 2014, 4:49 PM, I received a telephone call from Sam Myers of Antero Resources notifying me of a release of approximately 11 barrels of water based drilling fluid on containment. Mr. Myers also indicated that a small amount of the material was released over the site berm in the form of atomized mist. My inspection today of the site finds the spill on containment has been cleaned. I tested a small puddle, off site near the area of the release, with a low range Quantab and found the water to contain 193 parts per million total dissolved chlorides.
