The production equipment consisted of eight gas production units, eight 400 barrel tanks, two vapor recovery towers, and two combustors. The pipeline tie in was located on the West side of the pad. The identification was located at the wellhead and the production records were current.

The tank battery consists of eight 400 barrel tanks set inside of a plastic lined steel containment system. On the opposite side of the pad there are eight production units, also set in containment. Several pieces of construction equipment are parked on the location and are being used for the reclamation of the adjacent fresh water impoundment.
0484846846 2019-01-31 Production Wells No Violations On the day of my inspection accompanied by ODNR inspector Bill Stolarik I witnessed that the well was equipped with 5.5-inch diameter production casing and 2.875-inch diameter tubing that appeared to be open flowing. At the time of my inspection annular pressure monitoring was in place on the 9.625-inch diameter deep intermediate casing, and 13.375-inch diameter intermediate casing. Production equipment included a Gas Processing Unit, as well as 8- 400-barrel steel production fluid storage tanks, and a combustor that was shared with the other producing wells on the pad. There was secondary containment around the production fluid tanks, and the Gas Processing Units. The well pad was contained within an earthen dike, there were multiple Emergency Shutoff Devices on location. There was a midstream valve site on the west side of the pad. As well as a water impoundment to the south, right side of the lease road as you enter the pad. Identification tags were present, and production was up to date.

1169614259 2014-04-26 Casing String Witnessed No Violations My inspection today finds the Express Energy Services drilled 100 of 32 hole with an auger. Express then ran 100 of 26 casing. Express also cemented the casing by circulating 330 sks of class A cement with a 1.18 yield and a weight of 15.6 pounds. The cement stayed at surface.

1783109821 2014-05-07 Casing String Witnessed No Violations My inspection finds Patterson rig 332 ran 451 of 18.625 casing. I tested the mix water with a Quantab, the water did not contain elevated levels of chlorides. Allied cemented the casing in place with 905 sks of class A cement. I verified the cement weight with scales at 15.7 pounds per gallon.
