0216051084 2015-12-18 Production Wells No Violations My inspection today finds the well has been returned to production. All of the equipment that was damaged in the fire has been removed from location. The drainage sumps have been cleaned and tested. The previous compliance notice has been resolved.
0277619499 2017-01-18 Production Wells No Violations The purpose of my inspection is a status check on a producing well. The pad name is Warner and there are a total of 7 wells on this pad. During my inspection the well was equipped with a production tree style wellhead. The production equipment for this well consists of ten 400 barrel tanks within containment, and seven gas processing units. The vapor recovery system consists of two compressors and three Cimarron combustors. The production records are current, the identification is located at the wellhead, and there are no violations present.
0283994270 2014-06-07 Casing String Witnessed No Violations My inspection finds the 30 conductor hole was drilled to a depth of 100. One hundred feet of 26 casing was welded together and ran to bottom. Express Well Services cemented the casing in place with 230 sks of class A cement.
0369539035 2015-04-23 Completion Testing No Violations My inspection today finds U.S. Well Service is providing hydraulic stimulation services. Sixty perforations were placed between 16,263 and 16,410. All volumes and pressures were recorded in the stimulation report.
0593262483 2015-11-17 Production Wells Violations Noted I am onsite to meet with Antero personnel to determine the source of the fire and the extent of the environmental impact. Mike Stellfox is onsite for the Fire Marshalls office. After examining the area I determined an area approximately 120 by 25 has areas of visible pooling of glycol on the surface of the ground. According to the company the entire pad was cement solidified and there are no under drains around the location. A 2 XXL heavy wall pipe nipple at the base of the high pressure shut off valve (motor valve) was found with a large wash out in the side of it. Antero personnel believe the pipe failure may be due to sand in the system. The company has submitted a plan to remove impacted soils and recover all free product.
0607918819 2014-06-10 Drill / Deepen / Reopen No Violations I am accompanied by Brandon Trigg. Our inspection today finds the drilling rig, Patterson 343, is drilling 17.5 hole at a depth of 940. The anticipated total depth for this string is 1860.
0857259462 2016-06-17 Production Wells No Violations My inspection today finds the well is equipped in a manner consistent with being capable of production. The tank battery consists of twelve 400 barrel tanks set inside of a plastic lined steel containment system. Additionally, there are seven production units (GPUs) also set inside of a plastic lined steel containment system. Three Cimarron combustors and two compressors make up the vapor recovery system. A Meyers Well Service truck is on location hauling brine at the time of inspection.

The purpose of my inspection today was a status check on a production well. The pad was named Warner and it consisted of 7 wells. During my inspection the well was equipped with a production tree style wellhead with production valves in the open position. The production equipment consisted of ten 400 barrel tanks, 7 gas production units, one vent scrubber, and 2 combustors. The natural gas pipeline tie in was located on the Northwest side of the pad with no issues present. The identification was located on the wellhead and production records were current.

The drainage sumps are still closed. I requested the disposal manifests and the wellhead pressure test. The site is staffed by a Warrior employee 24 hours a day.

The production equipment consisted of tin 400 barrel tanks, 7 gas production units, and 2 combustors. The natural gas pipeline tie in was located on the Northwest side of the pad. The identification was located on the wellhead.

1402302417 2014-06-09 Casing String Witnessed No Violations My inspection today finds Patterson Drilling drilled 380 of 24 hole. The rig ran 376 of 18.625 casing. Allied cemented the casing in place with 726 sks of class A cement. I witnessed 60 barrels of cement return to surface.
1447889973 2015-11-18 Production Wells No Violations I am accompanied by Scott King and Trisha Cartmell. My inspection finds that all standing glycol has been removed. 3200 gallons of glycol were recovered from the production units. All wells on the pad remain shut in at this time. Once insurance adjusters are able to inspect the site the damaged units will be removed and repairs will resume.
1448064759 2015-11-20 Production Wells No Violations My inspection today finds the well is shut in. Well Service Group is on site and rebuilding the containment around the four productions units that were not affected by the fire. No glycol is visible on the ground. All drainage sumps are still closed and locked.
1470229432 2014-06-17 Drill / Deepen / Reopen No Violations Patterson rig 343 is drilling 12.25 hole at a depth of 6250. The location is well vegetated and erosion controls are in place.
1999691521 2015-07-30 Preliminary Restoration No Violations My inspection today finds the well site meets the preliminary restoration requirements of 1509.072(A). Well Service Group is onsite removing matting boards. The well has not been connected to production pipelines. The lease road has recently been treated with a dust suppression product.
2130937444 2015-11-16 Production Wells No Violations At 9:40 pm I received a call from Scott King stating that multiple fires were reported at the well site. The Caldwell VFD, Summerfield VFD and Quaker City VFD were on site upon my arrival at 10:15 pm. The fire was out at the time of my arrival. I found the production units for the Kahrig 3H, Daily 1H and the Daily 2H were damaged by fire. I walked the perimeter of location and checked the drainage sumps. The drainage sumps were closed and access to the valves was locked. Antero shut the sump last drain sump at 9:54 pm. Antero staffed the location with two security personnel to secure the well pad. At 11:03 I notified Sam Myers and Arron Goddard that Antero is in violation of 1509.22(A).
