Falcon rig 27 drilled 380 of 24 hole and ran 367 of 18.625 inch 87.5 pound casing. Allied Oil and Gas Services circulated 757 sacks of class A cement into place. The cement mix water tested at 101 ppm chlorides on a low range Quantab.
0699812004 2015-08-03 Preliminary Restoration No Violations My inspection today finds the well site meets the preliminary restoration requirements of 1509.072(A). The well is produced into a tank battery comprised of fourteen 400 barrel tanks set inside of a plastic lined steel containment.
0736721283 2016-01-13 Production Wells No Violations My inspection today finds the well is equipped in a manner consistent with being capable of production. The tank battery is comprised of fourteen 400 barrel tanks set inside of a plastic lined, liquid tight steel containment system. There are four incinerators set at the location. Seven other wells are located on the pad and share the production equipment. The ground is covered by three inches of snow at the time of inspection.
0980603213 2017-08-07 Production Wells No Violations The purpose of my inspection today was a status check on a production well. The pad was named Ervin and it consisted of 8 wells. During my inspection the well was equipped with a production tree style wellhead with production valves in the open position. The production casing had a gauge present reading 243 pounds per square inch and the production tubing had a gauge present reading 168 pounds per square inch. The production equipment consisted of fourteen 400 barrel tanks, 8 gas production units, 4 compressors, and 2 combustors. The natural gas pipeline tie in was located on the south side of the pad. The identification was located on the well and the 400 barrel tank.
1095806256 2015-07-10 Production Wells No Violations Status check inspection as a follow up to a on site accident that occurred on 07-08-2015. Upon inspection on this day I met with Anteros HSE representative Chris Warnock and Myers Well Service Regional Safety officer John Moore who indicated the employee of Myers Well Service was injured when a 3 line used in the production fluid transfer process was attempted to be disconnected from the water truck while under pressure causing the line to release from its connection and struck the employee in the face causing lacerations and fractures to the face. The line was free from fluid and no well bore related product was released. The company(Antero) has implemented a safety stand down until the incident can be investigated and corrections can be made to further avoid any future incidents. Addressing the well location a total of 8 wells were found on site and in production with a total of 14 production tanks present with a total capacity of 400 barrels each. All tanks were found inside of a poly lined metal construction containment dike area. Also on site there is a total of 8 gas processing units one supporting each existing well on the location. With proper well identification posted at the well head. Myers Well Service Ohio UIC # 25-427 water trucks were on location along with Ohio Oil Gathering hauling production fluid from the site.
1402937165 2014-06-16 Casing String Witnessed No Violations My inspection today finds Falcon rig 27 drilled a 17.5 hole to a depth of 1905. The rig crew ran 1870 of 13.625 casing. Allied cemented the casing in place with 1427 sacks of class A cement. I witnessed 22 barrels of cement to surface.
1419692932 2014-11-17 Drill / Deepen / Reopen No Violations My inspection today finds the drilling rig, Patterson 340, is building the bottom hole assembly to begin drilling the 8.75 curve and lateral.
1932395252 2017-01-25 Production Wells No Violations The purpose of my inspection is a status check on a producing well. The pad name is Ervin Pad and there are a total of 8 wells on this pad. During my inspection the well was equipped with a production tree style wellhead. The production equipment for this well consists of 14 400 barrel tanks within containment, and eight gas processing units. The vapor recovery system consists of two compressors and four Cimarron combustors. The production records are current, the identification is located at the wellhead, and there are no violations present.
2055553061 2015-06-03 Completion Testing No Violations My inspection today finds the service contractor, Fortis, has drilled out 32 completion plugs and are currently drilling plug number 33. Fire extinguishers are located around location as required.
