0281208218 2017-08-07 Production Wells No Violations The purpose of my inspection today was a status check on a production well. The pad was named Ervin and it consisted of 8 wells. During my inspection the well was equipped with a production tree style wellhead with production valves in the open position. The production casing had a gauge present reading 308 pounds per square inch and the production tubing had a gauge present reading 167 pounds per square inch. The production equipment consisted of fourteen 400 barrel tanks, 8 gas production units, 4 compressors, and 2 combustors. The natural gas pipeline tie in was located on the south side of the pad. The identification was located on the well and the 400 barrel tank.
0553488675 2015-08-03 Preliminary Restoration No Violations My inspection today finds the well site meets the preliminary restoration requirements of 1509.072(A). The well is produced into a tank battery comprised of fourteen 400 barrel tanks set inside of a plastic lined steel containment.
0791973948 2015-07-10 Production Wells No Violations Status check inspection as a follow up to a on site accident that occurred on 07-08-2015. Upon inspection on this day I met with Anteros HSE representative Chris Warnock and Myers Well Service Regional Safety officer John Moore who indicated the employee of Myers Well Service was injured when a 3 line used in the production fluid transfer process was attempted to be disconnected from the water truck while under pressure causing the line to release from its connection and struck the employee in the face causing lacerations and fractures to the face. The line was free from fluid and no well bore related product was released. The company(Antero) has implemented a safety stand down until the incident can be investigated and corrections can be made to further avoid any future incidents. Addressing the well location a total of 8 wells were found on site and in production with a total of 14 production tanks present with a total capacity of 400 barrels each. All tanks were found inside of a poly lined metal construction containment dike area. Also on site there is a total of 8 gas processing units one supporting each existing well on the location. With proper well identification posted at the well head. Myers Well Service Ohio UIC # 25-427 water trucks were on location along with Ohio Oil Gathering hauling production fluid from the site.
0926793559 2014-06-18 Casing String Witnessed No Violations I am onsite today to witness the surface cementing operations. My inspection finds Falcon 27 drilled a 24 hole to a depth of 374. The rig then ran 367 of 18.625 casing. The casing was cemented in place by Allied Oil and Gas Services. Allied circulated 723 sacks of class A cement into place with 52 bbl returned to surface.

1636847650 2018-06-14 Production Wells No Violations Status check inspection accompanied by ODNR North Region Emergency Response Coordinator Andrew Adgate. It was observed on this day that a total of 8 wells are present on the pad with proper well identification posted. To support the 8 well there are 8 gas processing units and a total of 14 production tanks situated inside a polymer lined steel containment dike area.
1900167965 2017-01-25 Production Wells No Violations The purpose of my inspection is a status check on a producing well. The pad name is Ervin Pad and there are a total of 8 wells on this pad. During my inspection the well was equipped with a production tree style wellhead. The production equipment for this well consists of 14 400 barrel tanks within containment, and eight gas processing units. The vapor recovery system consists of two compressors and four Cimarron combustors. The production records are current, the identification is located at the wellhead, and there are no violations present.
