During my inspection the well was equipped with a production tree style wellhead with valves in the open position. Annular pressure monitoring was present. The production equipment consisted of five gas production units, eight 400 barrel tanks, two compressors, and three combustors. Identification was present and production records were current.

The water sumps and drainage system on the perimeter of the location has been recently cleaned and repaired.

The tank battery consists of eight 400 barrel tanks set in a plastic lined liquid tight steel containment. The tank battery is shared by four other wells on the pad. Two vapor recovery compressors and three incinerators are located near the entry of the well pad. Heavy rain is falling at the time of inspection.
1080609344 2014-08-03 Casing String Witnessed No Violations My inspection today finds Falcon Drilling rig 27 ran 2073 of J-55 13.375 casing. Allied Oil and Gas Services circulated class A cement into place. I witnessed good cement returns at surface. The cement weight was verified by pressurized scale at 15.7 pounds.
1147248058 2017-10-18 Production Wells No Violations The purpose of my inspection was a status check on a production well. The well pad name is Cynthia and it consists of 5 wells. The well was equipped with a production tree style wellhead with a plunger lift system. Production valves were in the open position. Annular pressure monitoring was present. The gauge on the production casing read 380 pounds per square inch and the gauge on the production tubing read 180 pounds per square inch. The production equipment consisted of eight 400 barrel tanks within steel line containment and five gas production units. A vapor recovery system consisted of two compressors, one vent scrubber, and three combustors. The pipeline tie in was located on the North side of the pad. A dust suppressant had been recently applied to the lease road. The identification was located at the wellhead and the production records were current.

1309703163 2016-01-13 Production Wells No Violations My inspection today finds the well is equipped in a manner consistent with being capable of production. The tank battery is comprised of eight 400 barrel tanks set inside of a plastic lined, liquid tight steel containment system. There are three vapor recovery units and three incinerators set at the location. Four other wells are located on the pad and share the production equipment. The ground is covered by three inches of snow at the time of inspection.
1438811709 2014-07-31 Casing String Witnessed No Violations My inspection today finds the drilling rig, Falcon Drilling 27, ran 374 of 18.625 casing with four centralizers and a float shoe. Allied Oil and Gas Services cemented the casing in place with 801 sacks of class A cement. A test of the mix water with a low range Quantab found the chloride levels were at 40 ppm.

The tank battery is comprised of eight 400 barrel tanks set inside of a plastic lined steel containment system. There are no personnel onsite at the time of inspection.
1655795431 2014-07-24 Casing String Witnessed No Violations I am on site to witness the conductor cementing operations. Express Energy Services drilled to a total depth of 80. The top 25 was 42 hole with bottom 55 being 32 hole. Express cemented 80 of 26 weld casing in place with 430 sacks of class A cement. I witnessed 18 barrels of returns.
1854532115 2018-11-27 Production Wells No Violations On the day of my inspection I witnessed that the well was equipped with a four-string casing design with 5.5-inch diameter production casing and 2.875-inch diameter tubing equipped with a plunger lift system. At the time of my inspection annular pressure monitoring was in place, and the 13.375-inch diameter casing had 320 pounds per square inch, the 9.625-inch diameter casing had 0 pounds per square inch. Production equipment included a Gas Processing Unit, as well as 8- 400-barrel steel production fluid tanks, and 2 combustors that were shared with the other producing wells on the pad. There was secondary containment around the production fluid tanks, and the Gas Processing Units. There was a midstream valve site on the northwest side of the pad. The well pad was contained within an earthen dike, there were multiple Emergency Shutoff Devices on location. Identification tags were present, and production was up to date.
