1419886851 2014-12-29 Casing String Witnessed No Violations I am onsite to witness the casing installation and cementing operations of the conductor casing string. Express Energy Services drilled 100 of 36 hole with an auger rig. Express then ran 100 of 30 .375 wall weld casing in the well. The cement mix water was tested with a low range Quantab and registered 82 parts per million total chlorides. The casing was cemented by circulating 275 sacks of class A cement with .5 pounds per sack flowseal and 2% CaCl into the annular space. I witnessed 11 barrels of cement return to surface and remain static. One foot of cement was left in the cellar to ensure a liquid tight seal.
1498567838 2017-06-27 Final Restoration No Violations The purpose of my inspection is a status check on a final restoration. During my inspection all equipment belonging to the plugged well had been removed and the well site was left with a graded gravel surface. There was no evidence at surface of the well existing. Production equipment that remains on location is used for the four producing wells on the pad. The location has met 1509.072 (B) final restoration requirements.
1647552298 2016-07-22 Plug / Plug Back No Violations My inspection today finds a vac truck has removed the water from the wellbore. Caldwell Redi-mix poured 16 yards of concrete into the wellbore and filled it to surface. The cement stayed at surface. The company was instructed to place a steel plate with the permit number on top of the casing string.
