0065082713 2017-09-28 Completion Testing No Violations The purpose of my inspection was to investigate complaint number 5709. The route taken to enter the pad consisted of taking State Route 147 to Mud Run road and then on to the lease road. Roadways were free from debris. During my inspection Hydraulic Stimulation Operations were staged to take place. US Well Services, Allied Horizontal Wireline, and Antero were all on location. Allied Horizontal Wireline were rigged up to the Walsh 1H permit number 3412124538 to perforate stage 8 of 55 and US Well Services was rigged up to the Iles 1H permit number 3412124539 to prepare and Hydraulically Stimulate stage 21 of 54. No sirens were heard from the pad and there were not any issues on location.
0160296408 2018-01-09 Completion Testing No Violations The pad is named Bond and it consists of 9 wells. The purpose of my inspection was a status check. During my inspection Fortis Energy Services was on location with their service rig drilling out frac plugs on the Iles 2H, 3412124540. This well was equipped with a production tree style wellhead with valves in the closed position. Frac plugs have recently been drilled out and the well is prepared to be flowed back. The production equipment consisted of nine gas production units, fifteen 400 barrel tanks, two compressors, and four combustors.

0246485652 2017-06-06 Completion Testing No Violations The pad is named Bond and it consists of 9 wells. The purpose of my inspection was a status check. During my inspection the well was equipped with a Frac valve. The production equipment on the pad consisted of nine gas production units, fifteen 400 barrel tanks, two compressors, and four combustors. Containment was present on the pad with sound walls surrounding the pad as Antero prepares the location for Hydraulic Fracturing operations on the 5 of wells.
0346389654 2014-11-19 Casing String Witnessed No Violations I am onsite to witness the surface casing installation and cementing operations. Sidewinder Drilling rig 54 ran 417.5 of 18.625 casing inside a 24 borehole. Allied Oil and Gas Services cemented the casing in place with 173 barrels of 15.6 pound class A cement. The pre job water test indicated that the chlorides were at 43 parts per million (1.6 on a low range Quantab). I witnessed 70 barrels of cement circulate to surface. The casing pressure test was passed.
0563277574 2017-03-23 Preliminary Restoration No Violations The pad is named Bond and it consists of 8 wells. The purpose of my inspection was a preliminary restoration. During my inspection Antero was on location with contractors present to nipple up the wellhead and get it tied into production lines. The area affected by the well pad construction has all been restored. There are no slips or erosion issues on location. The production equipment on the pad consisted of eight gas production units, fifteen 400 barrel tanks, two compressors, and two combustors. Location has met 1509.072 A requirements.
0976275655 2017-02-02 Drill / Deepen / Reopen No Violations On the day of my inspection Patterson UTI Drilling Rig #342 was drilling their lateral with an 8.75 inch diameter bit at 16114 feet on 10.5 pounds per gallon mud simulating 12.3 pounds per gallon with 3000 units of gas. Patterson UTI Drilling Rig #342 is planning on drilling to projected total depth of 19088 feet with 5.5 inch diameter casing and cementing to follow. I completed walk around with Antero Consultant Josh Scharf revealing proper containment was in place and an intermittent 1 foot flare.
1219761653 2017-09-26 Completion Testing No Violations The purpose of my inspection was to investigate complaint number 5709. During my inspection Hydraulic Stimulation Operations were taking place. US Well Services, Allied Horizontal Wireline, and Antero were all on location. Allied Horizontal Wireline were rigged up to the Walsh 1H permit number 3412124538 to perforate stage 4 of 55 and US Well Services was rigged up to the Iles 1H permit number 3412124539 to prepare and Hydraulically Stimulate stage 17 of 54. I completed a walk around on location and found there to be no issues. I spoke with the company man on location and he stated that there had been no issues with operations.

2078712109 2018-03-28 Production Wells No Violations At 3:28 p.m. on 3/27/18 Antero Resources Production Supervisor Doug Stubbins called and reported some electrical issues that they were having with their combustor. Mr. Stubbins stated that the electric cable on the combustor burnt up and caused small flame that was extinguished immediately. The pad is named Bond and it consists of 9 wells. The purpose of my inspection was a status check. During my inspection the well was equipped with a production tree style wellhead with valves in the open position. Pressure monitoring was present on all casing strings. The production equipment consisted of nine gas production units, fifteen 400 barrel tanks, two compressors, and four combustors. Identification was present and production records were current. Antero well tender was on location.
