0827362309 2016-07-20 Casing String Witnessed No Violations On site inspection to witness the surface casing cement job found Frontier Drilling rig # 32 rigged up on the well and has drilled a 24 surface bore hole to a measured depth of 541and ran 531of 54.5 lb/ft J-55 casing with a guide shoe and float collar ran and proper centralizer placement demonstrated. Halliburton cement service cemented the casing using 880 sacks of class G cement with the cement weight verified by weight of scale at 15.6 lb/gl, circulation was established with 250 barrels of fresh water followed by 30 barrels of 6% gel before cement. Cement returns were witnessed to surface and the casing was pressure tested to 500 psi over displacement pressure.
0855329940 2018-11-19 Production Wells No Violations On the day of my inspection I witnessed that the well was equipped with 5.5-inch diameter production casing and 2.875-inch diameter tubing that appeared to be open flowing. At the time of my inspection annular pressure monitoring was in place, and the 13.375-inch diameter casing had 200 pounds per square inch, the 9.625-inch diameter casing had 0 pounds per square inch. Production equipment included a Gas Processing Unit, as well as 12- 400-barrel steel production fluid tanks, and 2 combustors that were shared with the other producing wells on the pad. There was secondary containment around the production fluid tanks, and the Gas Processing Units. The well pad was contained within an earthen dike, there were multiple Emergency Shutoff Devices on location. There was a midstream valve site near located on the northwest corner of the pad. Identification tags were present, and production was up to date.
0918897911 2017-03-06 Preliminary Restoration No Violations Preliminary Restoration status check inspection accompanied by ODNR County Inspector Austin Gutridge. The area affected by the well pad construction meets ORC 1509.072 with vegetation established and erosion control in place. The lease road is stoned and in good repair. Signage is posted at the well pad entrance. U.S Well service crews are on site completing well stimulation operations on the 5 active wells on the pad.
1034545767 2015-10-14 Casing String Witnessed No Violations I am onsite to witness the conductor casing cementing operations. Express Energy Services drilled a 32 borehole to a depth of 100. Express then ran 100 of 26 weld casing. Express cemented the casing in place with 330 sacks of class A cement. I witnessed 19 barrels of cement circulate to surface.
1355656936 2017-07-11 Production Wells No Violations The pad is named Fuller and it consists of five production wells. The purpose of my inspection was a status check on a production well. During my inspection the well was equipped with a production tree style wellhead with production valves in the open position. The production equipment consisted of twelve 400 barrel tanks, five sand traps, five gas production units, three compressors, and three combustors. Myers water truck was on location loading produced fluids.
1384177360 2016-07-19 Drill / Deepen / Reopen No Violations On site inspection accompanied by members of the Divisions Engineering and Radiology staff who was given a operation tour of the drilling rig back yard solidification area. Frontier Drilling rig # 32 is on location and has began drilling the 24 surface borehole on air.
1469002844 2016-07-20 Casing String Witnessed No Violations I am onsite to witness the surface casing cementing operations. Frontier Drilling Rig 32 drilled a 541 twenty four inch diameter borehole. The rig then installed 531.7 of 18.625 J-55 casing utilizing four centralizers to ensure proper stand off. Halliburton had difficulty installing the cement head. I requested assistance from Inspector Stolarik to watch the cement emplacement.
