0133048585 2018-11-19 Production Wells No Violations On the day of my inspection I witnessed that the well was equipped 5.5-inch diameter production casing and 2.875-inch diameter tubing that appeared to be open flowing. At the time of my inspection annular pressure monitoring was in place, and the 13.375-inch diameter casing had 200 pounds per square inch, the 9.625-inch diameter casing had 0 pounds per square inch. Production equipment included a Gas Processing Unit, as well as 12- 400-barrel steel production fluid tanks, and 2 combustors that were shared with the other producing wells on the pad. There was secondary containment around the production fluid tanks, and the Gas Processing Units. The well pad was contained within an earthen dike, there were multiple Emergency Shutoff Devices on location. There was a midstream valve site near located on the northwest corner of the pad. Identification tags were present, and production was up to date.
0184748650 2016-07-18 Casing String Witnessed No Violations I am onsite to witness the second intermediate casing installation and cementing operations. Frontier Rig 32 drilled 7109 of 12.25 borehole. Then, the rig installed 7094.4 of HCP-110 casing weighing 47 pounds per foot. The casing was centralized on every third joint. Cement baskets were placed at 2414 and 2570. Halliburton cemented the casing inplace with 430 sacks of HalCem and 830 sacks of ExpandaCem. Twenty eight barrels of cement circulated to surface.
0325833206 2017-04-14 Completion Testing No Violations The purpose of my inspection was a status check on drilling out frac plugs from prior well stimulation operations. Fortis Service rig equipped with a snubbing unit, Power Swivel trailer, and Hydraulic Pipe Wrangler Trailer are on location. The rig is a using 2.875 inch diameter workstring with PH 6 connection weighing 8.7 pound per foot to drill out plugs and they well run a 2.875 inch diameter grade N 80 production tubing. Containment was placed under the rig and frac tanks. A pad walk around inspection found no violations present.

1041153325 2017-07-11 Production Wells No Violations The pad is named Fuller and it consists of five production wells. The purpose of my inspection was a status check on a production well. During my inspection the well was equipped with a production tree style wellhead with production valves in the open position. The production equipment consisted of twelve 400 barrel tanks, five sand traps, five gas production units, three compressors, and three combustors. Myers water truck was on location loading produced fluids.

1444910603 2015-10-15 Casing String Witnessed No Violations The said well was spudded on 10/15/2015 by Express Energys top hole rig with a 100 x 32 conductor borehole being drilled and 100 of 26 conductor being set. Express Energys Cement Service cemented the casing using 330 sacks of class A cement with the cement displaced to 90 with 55.74 barrels of fresh water and good cement returns witnessed to surface.
1468279440 2016-07-11 Casing String Witnessed No Violations 07/11/2016 -On site inspection to witness the 18.625 surface casing cement job found Frontier Drilling rig # 32 on the hole with a 24 surface borehole drilled to 541 and 531 of J-55 87.5 lb/ft Buttress thread casing ran with a guide shoe - float collar and 4 casing centralizers ran for casing equipment. Halliburton Cement Service cemented the casing using 850 sacks of Class G cement with circulation established with 250 barrels of fresh water followed by 30 barrels of 6% gel spacer. Circulation was maintained throughout the cement job and cement returns were witnessed to surface with the casing pressure tested to 792 psi or 500 psi over cement displacement pressure.
1619121771 2017-07-11 Preliminary Restoration No Violations The purpose of my inspection is a status check on a preliminary restoration. During my inspection the area that has been disturbed by pad construction has been restored. All drilling supplies and equipment had been removed from location. The pad had a gravel surface with thick vegetation growth on the berm surrounding the pad. No erosion existed on the pads perimeter. The location has met 1509.072(A) requirements for preliminary restoration.

2023301524 2016-07-13 Production Wells No Violations I am onsite to witness the cementing operations of the first intermediate casing string. Frontier Drilling rig 32 installed 1829.5 of J-55 54.5 pound casing with the assistance of Wyoming Casing power tongs. Halliburton cemented the casing in place by utilizing 272 barrels of HalCem at a weight of 15.6 pounds per gallon. I witnessed 30 barrels of cement circulate to surface. I witnessed the casing integrity test and verified the casing held pressure.
