0434695334 2017-07-11 Production Wells No Violations The pad is named Fuller and it consists of five production wells. The purpose of my inspection was a status check on a production well. During my inspection the well was equipped with a production tree style wellhead with production valves in the open position. The production equipment consisted of twelve 400 barrel tanks, five sand traps, five gas production units, three compressors, and three combustors. Myers water truck was on location loading produced fluids.
0900787570 2018-11-19 Production Wells No Violations On the day of my inspection I witnessed that the well was equipped with 5.5-inch diameter production casing and 2.875-inch diameter tubing that appeared to be open flowing. At the time of my inspection annular pressure monitoring was in place, and the 13.375-inch diameter casing had 240 pounds per square inch, the 9.625-inch diameter casing had 150 pounds per square inch. Production equipment included a Gas Processing Unit, as well as 12- 400-barrel steel production fluid tanks, and 2 combustors that were shared with the other producing wells on the pad. There was secondary containment around the production fluid tanks, and the Gas Processing Units. The well pad was contained within an earthen dike, there were multiple Emergency Shutoff Devices on location. There was a midstream valve site near located on the northwest corner of the pad. Identification tags were present, and production was up to date.
0958225989 2015-10-13 Casing String Witnessed No Violations I am onsite to witness the conductor casing cementing operations. Express Energy Services drilled a 32 borehole to a depth of 100. Express then ran 100 of 26 weld casing. Express cemented the casing in place with 330 sacks of class A cement. I witnessed 22 barrels of cement circulate to surface.
0978766351 2016-09-09 Drill / Deepen / Reopen No Violations My inspection today finds the rig drilled to a depth of 8259 when the manifold began to leak. The drill pipe was pulled into the casing and manifold is being repaired.
1442903461 2017-03-06 Preliminary Restoration No Violations Preliminary Restoration status check inspection accompanied by ODNR County Inspector Austin Gutridge. The area affected by the well pad construction meets ORC 1509.072 with vegetation established and erosion control in place. The lease road is stoned and in good repair. Signage is posted at the well pad entrance. U.S Well service crews are on site completing well stimulation operations on the 5 active wells on the pad.
1467505443 2016-07-02 Casing String Witnessed No Violations 07/02/2016- On site inspection at 17:30 pm to witness the surface casing cement job found Frontier Drilling rig # 32 on location and a 24 surface borehole drilled to 532. The rig ran 531 of 18.625 J-
1467731916 2016-07-05 Casing String Witnessed No Violations I am onsite to witness the first intermediate casing installation and cementing operations. The drilling contractor, Frontier rig 32, drilled 1890 of 17.5 hole. While drilling they encountered a significant flow of water from the Big Injun formation. 1873 of 13.375 J-55 54.5 pound per foot casing was installed. Haliburton cemented the casing in place with 180 barrels of HalCem at a weight of 15.6 pounds per gallon. I witnessed 57 barrels of cement circulate to surface. The cement mix water was tested for chlorides and PH prior to commencement of the job.
1467746510 2016-07-05 Brine Hauler No Violations Received call from inspector Bill Stolarik informing me of brine truck roll over on St. Johns road in Beaver twp,on arrival at crash site it was noted that Buckeye Water Services vacuum truck had rolled over and lost approximately 25 bbl. Of water and Myers truck pulled remaining product from truck and went on to drill site to fill his tanker truck and haul to a approved disposal site.

Note: Cause of wreck was two trucks tried to pass on narrow road and one truck got onto soft shoulder of road and loss control of truck.oepa was called in to investigate scene and the divisions Emergency Response team was also notified of incident.
