The area affected by the well pad construction meets ORC 1509.072 with vegetation established and erosion control in place. The lease road is stoned and in good repair. Signage is posted at the well pad entrance. U.S Well service crews are on site completing well stimulation operations on the 5 active wells on the pad.

0657458719 2018-11-19 Production Wells No Violations On the day of my inspection I witnessed that the well was equipped with 5.5-inch diameter production casing and 2.875-inch diameter tubing that appeared to be open flowing. At the time of my inspection annular pressure monitoring was in place, and the 13.375-inch diameter casing had 300 pounds per square inch, the 9.625-inch diameter casing had 260 pounds per square inch. Production equipment included a Gas Processing Unit, as well as 12- 400-barrel steel production fluid tanks, and 2 combustors that were shared with the other producing wells on the pad. There was secondary containment around the production fluid tanks, and the Gas Processing Units. The well pad was contained within an earthen dike, there were multiple Emergency Shutoff Devices on location. There was a midstream valve site near located on the northwest corner of the pad. Identification tags were present, and production was up to date.
0667796216 2015-10-14 Completion Testing No Violations I am onsite to witness the conductor casing cementing operations. Express Energy Services drilled a 32 borehole to a depth of 100. Express then ran 100 of 26 weld casing. Express cemented the casing in place with 330 sacks of class A cement. I witnessed 18 barrels of cement circulate to surface.

During my inspection US Well Services, Fortis Services, Great White Services, and Antero were all on location. The company man for Antero stated they were preparing to trip into hole with tubing conveyed perforating guns.

1474037326 2016-09-16 Casing String Witnessed No Violations I am onsite to witness production casing cementing operations. C&J Energy Services cemented the casing in place with 232 sacks of class H based lead cement and 2022 sacks of class H tail cement. I witnessed C&J personnel verify cement weight with pressurized cement scales. Due to time constraints I left location prior to completion of the job. The cement calculations place the estimated cement top at 6282.
1483482229 2017-01-03 Completion Testing No Violations Status check inspection on the well completion process found US Well Service pressure pumping standing by on location and a service rig owned by Fortis Services rigged up on the hole supported by a tubing snubbing unit owned by Great White Services. The company man on location reported that a wireline perforating gun became stuck in the 5.5 diameter production casing at a measure depth of 20,200 which was 112 short of their target point of 20,312. The company was pumping down stage 6 to be perforated with stages 1 through 5 perforated and stimulated. In their efforts to free the lodged tool the company deployed the composite bridge plug attached to the gun with efforts unsuccessful. With the perforating gun lodged in the casing the wireline was severed approximately 700 from the gun sometime around the 12th of December 2016. The service rig has made several unsuccessful attempts to recover the lost perforating gun and wireline in the hole and has requested to abandon the lost tool and 900 of horizontal lateral beyond the tool and restart completion operations by setting a completion plug at 20,137 and starting perforating at 21,124.
1499949405 2017-07-11 Production Wells No Violations The pad is named Fuller and it consists of five production wells. The purpose of my inspection was a status check on a production well. During my inspection the well was equipped with a production tree style wellhead with production valves in the open position. The production equipment consisted of twelve 400 barrel tanks, five sand traps, five gas production units, three compressors, and three combustors. Myers water truck was on location loading produced fluids.
